New mom recovering from flesh-eating bacteria is a 'fighter'


Lana Kuykendall, the South Carolina woman struck by flesh eating bacteria shortly after delivering twins, appears to be well on the road to recovery.
"It's been a long road, a lot of which I don't remember, but we're recovering and that's what matters," Kuykendall told Willie Geist in an exclusive interview on TODAY.
She's moved from a hospital to a rehabilitation facility, and she could go home as early as this week. Last month, she was able to hold her babies for the first time since being infected, and she's now able to walk. Her friends and husband Darren say the babies have been a huge motivator in her fast recovery.

"She's a fighter. She works hard every day and she pushes through it," her physical therapist, Lindsay Swift, tells TODAY.
After nearly 20 surgical procedures, the 36-year-old mom’s condition was upgraded to “good” by doctors at Greenville Memorial Hospital in late June.
"It was touch-and go each and every day, and it was stressful," said Darren, a firefighter. "She's very strong. Her faith in God has has really pulled her through -- pulled all of us through. I knew that she was a fighter from the word go, and I love her with all my heart. She's the best thing that ever happened to me."
On May 11, just days after her babies were born, Kuykendall noticed a dark spot on the back of her left leg. Thinking it might be a blood clot, Kuykendall, who is a paramedic, checked herself in to the hospital. Nhan Dan - Afternoon 16-7, the Veterinary Department of Quang Binh province, said test results on dead ducks in two districts of Le Thuy and Quang Ninh showed that H5N1 flu appears in Quang Binh.

(SGGPO.) - Day 16-7 Pham Hong Son, the Director of the Veterinary Department confirmed Quang Binh province, in An Ninh commune, Quang Ninh, Quang Binh Province samples from dead birds have side tested positive bird flu virus H5N1.

An Ninh People's Committee, said in recent days, in the village of Thong Nhat 300 ducks died series and the phenomenon has been reported to the Department of Veterinary, veterinary officials took samples tested duck deaths and detection of avian influenza broke out here.

Ireland-Six ‘Mad Cow’deaths in 3 years?

          

THE human variant of ‘Mad Cow Disease’ was mentioned on six Northern Ireland death certificates over the past three years, the Finance Minister Sammy Wilson has revealed.

The Minister provided details of the number of deaths registered in Northern Ireland between 2009 and 2011, where ‘Creutzfeldt-Jakob disease’ was mentioned on the death certificate.
Mr Wilson’s colleague the Health Minister Edwin Poots said that of 176 deaths from Creutzfeldt-Jakob disease across the UK since 1995, just three had occurred in Northern Ireland.
Mr Poots said none of the three deaths were due to blood transfusions and that it was impossible to say for sure if the three local deaths were due to the consumption of infected beef products.
“There have been 176 deaths in the United Kingdom from variant Creutzfeldt-Jakob disease (vCJD) since 1995. Three of these deaths were in Northern Ireland,” he stated.
“None of the three deaths in Northern Ireland was associated with blood transfusions. Possible importation of vaccine against avian influenza in Italy

Ukraine bans poultry imports from China

Ukraine has banned poultry imports from seven administrative territories of China because of outbreaks of highly pathogenic avian influenza (HPAI) on their territory.
Information about the ban was sent out in a telegram by State Veterinary Inspector Volodymyr Horzheyev, the press service of the State Veterinary and Biosecurity Service of Ukraine reported on Friday.

Cambodia, WHO wrap up probe of EV-71 infections

Jul 12, 2012 (CIDRAP News) – A joint investigation into dozens of puzzling lethal respiratory illnesses in Cambodian children concluded that the infections were caused by a severe form of hand, foot, and mouth disease (HFMD), the World Health Organization (WHO) and Cambodia's health ministry said today in a statement.


Earlier this week, the agencies said lab analysis of samples from several of the patients pointed to enterovirus 71 (EV-71), which can cause a severe form of HFMD in some patients.

In their wrap-up of the investigation, the groups said the findings include a review of hospital records on children treated at Kantha Bopha Children's Hospital, which reported unusual numbers of illnesses and deaths since April, and records from other hospitals.

The probe also included a review of lab tests, follow-up of affected families by rapid response teams, and analysis of disease data from Cambodia's surveillance system. The Pasteur Institute in Cambodia and the US Centers for Disease Control and Prevention (CDC) assisted with the investigation.

Officials identified 78 cases, including 62 initially reported from Kantha Bopha Hospital. Investigators focused on 61 cases that fit the case definition, which included 54 deaths. The Pasteur Institute tested samples from 31 patients, and most were positive for EV-71. A few were positive for other pathogens, including Haemophilus influenzae type B and Streptococcus suis, according to the statement.

Most of the patients were under age 3, and some were affected by chronic conditions and malnutrition. The children were from 14 different provinces.

Investigators found that many of the patients had been given steroids at some point in their illness, which can worsen the condition of patients with EV-71 infections, the statement said.

In response to the illnesses, Cambodia's health ministry, with WHO support, has launched enhanced surveillance for neurorespiratory syndrome, a key feature observed in some patients with severe HFMD caused by EV-71. Officials have asked health centers to also report patients with mild HFMD infections. The statement said officials expect to identify occasional new cases of the severe form of the disease in the months ahead.

Health officials are also developing guidelines and training courses for managing patients with both mild and severe HFMD.

The statement said HFMD is not a new disease in Cambodia, but it did not say if illnesses linked to EV-71 had been detected before.

HFMD is common in many Asian countries. A Jun 26 HFMD surveillance update from the WHO's Western Pacific Region office (WPRO) noted that seven countries or administrative regions have reported cases this year, most of them in China, Vietnam, and Singapore.

In other developments, health authorities in the Philippines on Jul 10 made EV-71 infection a reportable illness, which requires individuals and health facilities to report infections to public health officials, Rappler.com, a social news network based near Manila, reported yesterday. The country's department of health said yesterday that it would soon release guidelines on reporting EV-71 illnesses.

Philippines health officials said no travel restrictions are needed for people traveling to and from Cambodia, though officials are screening in-bound travelers at all points of entry, according to the report.

The first successful vaccines, like Jenner's smallpox vaccine and the first Salk vaccine against polio, were based on viruses that do not cause illness or severe symptoms. Vaccine development has since shifted largely to the use of proteins that are used by the disease-causing agents, but there are still some cases where a dead or attenuated virus is the most effective method of generating immunity.
The use of viruses for vaccines, however, has always come with a bit of a concern. When it comes to viruses, one-in-a-million events happen all the time, and evolution gives any viruses used in vaccines a lot to work with: many related viruses in the wild, and animal genomes that are littered with pieces of former viruses. Now, researchers have discovered a case where two different agricultural vaccines have recombined to create a new, virulent strain of the disease they were intended to prevent.
In poultry, a form of herpesvirus (gallid herpesvirus 1) causes a respiratory disease that is sometimes fatal; even if it doesn't kill the animals, it causes a reduced egg production. As a result, several vaccines have been developed against the virus responsible, based on attenuated forms that do not cause serious illness. Three of these vaccines are approved for use in Australia: two based on viral strains that are present in Australia, and a third developed against a strain common in Europe.

China Sends Medical Team to Treat Epidemic in Cambodia

A Chinese medical team has been sent to Cambodia to aid in alleviation of an outbreak of EV71, an intestinal virus, the Ministry of Health said Thursday.
The medical team arrived in Phnom Penh Thursday morning with medical supplies, the ministry said.
The ministry was informed by the World Health Organization (WHO) that a number of children have been infected by EV71 in several provinces of Cambodia since April, the statement said.
The Chinese team will assist Cambodian medical workers in analyzing the virus, treating patients, studying the epidemic and training local staff, the statement said.

A severe form of hand, foot and mouth disease (HFMD) was the cause of the majority of recent illnesses and deaths in children reported in Cambodia, according to a joint investigation by the country’s Government and the United Nations released today. Following reports from a children’s hospital of unusual numbers of illnesses and deaths among children hospitalized since April 2012, the Cambodian Ministry of Health, with support from the World Health Organization (WHO) and other partners, conducted an investigation which identified 78 cases of HFMD, 54 of which resulted in deaths.
HFMD is a common infectious disease of infants and children. It is spread through direct contact with the mucus, saliva, or faeces of an infected person. It typically occurs in small epidemics in nursery schools or kindergartens, usually during the summer and autumn months. Nearly all patients recover in 10 days without medical treatment, and complications are uncommon.
“The investigation revealed that most of the cases were under three years old, with some suffering with chronic conditions and malnutrition,” WHO and the Ministry of Health said in a joint news release. “The cases were from 14 different provinces and many of them were given steroids at some point during their illness. Steroid use has been shown to worsen the condition of patients with EV-71 virus, which causes HFMD.”
In response to this event, Cambodian authorities, with support from WHO, have enhanced surveillance to identify symptoms of the disease and are developing guidelines and training courses for management of HFMD patients.
The Government is also working on a campaign to raise awareness on prevention, identification and care of children with HFMD.
The disease – which is not new in Cambodia – can be prevented with good hygiene. Two cases of hand, foot and mouth disease (HFMD) have been detected in Surin following an outbreak in Cambodia which borders the northeastern province.

Deputy Public Health Minister Surawit Khonsomboon yesterday visited two children at Surin Hospital after they developed HFMD symptoms.

Doctors found the children were not in a severe condition. One is from Krasang district of Buri Ram and the other is from Surin's Muang district.

Mr Surawit said the HFMD outbreak in Cambodia has claimed the lives of more than 50 children, who were infected with Enterovirus Type 71 (EV-71), a dangerous strain.

The two HFMD-infected children in Surin were being quarantined at the public hospital to prevent the disease spreading, the deputy minister said.
Last year, six Thais died of HFMD.
Since January this year, a total of 10,813 people in Thailand have been infected with HFMD, but they were not infected with the EV-71 strain that claimed many lives in Cambodia. In Surin alone, a total of 40 HFMD cases have been reported this year.

The deputy minister said teams of health officials would be sent to border areas to check whether there were any more cases of HFMD.
He asked childcare centres and schools to close temporarily if up to five students were found to have caught the disease.
He said prevention measures have been put in place along the Thai-Cambodian border, particularly at Chong Chom permanent border checkpoint in Surin's Kap Choeng district.

He advised residents to wash their hands frequently as part of primary prevention. He instructed paediatricians at all hospitals, particularly in areas near the Thai-Cambodian border, to thoroughly examine diseases and health conditions of child patients.

PHNOM PENH, July 12 (Xinhua) -- The severe form of hand, foot and mouth disease (HFMD) was the cause of illnesses and deaths of 54 Cambodian children since April, concluded the findings of a joint investigation on Thursday.
The Ministry of Health, the World Health Organization, the Institut Pasteur du Cambodge and the U.S. Centers for Disease Control and Prevention conducted investigation following reports from Kantha Bopha Children's hospital of unusual numbers of illness and deaths among children hospitalized since April.
The investigation included a thorough review of the hospital records of the patients from Kantha Bopha hospital as well as other hospitals, laboratory tests, active follow-up with the affected families by the local Rapid Response Teams and evaluation of the data from national surveillance system, said a joint statement from the Ministry of Health and the WHO.
Based on the investigation, a total of 78 cases were identified. These included the initial 62 cases reported by Kantha Bopha hospital, and cases reported from other hospitals. Of these the investigation focused on 61 cases who fitted the criteria used ( the case definition), of which 54 had died.
It said that it was not possible to test all the patient as some of them died before appropriate samples could be taken. Samples from a total of 31 patients were obtained and tested for a number of pathogens by Institut Pasteur du Cambodge.
Of these, majority tested positive for Enterovirus 71 (EV-71) which causes hand, foot and mouth disease.
A small proportion of samples also tested positive for other pathogens including Haemophilus Influenzae type B and Streptococcus suis, it said.
The investigation revealed that most of the cases were below 3 years old with some suffering from chronic conditions and malnutrition.
The cases were from 14 different provinces and many of them were given steroids at some point during their illness. Steroid use has been shown to worsen the condition of patients with EV-71.
HFMD virus is contagious and infection in spread from person to person by direct contact with nose or throat discharges, saliva, fluid from blisters, or the stool of infected persons.
Good hygiene practice can prevent HFMD. Presently, there is no specific treatment available for HFMD, said the joint statement. The central province of Quang Nam has recorded nearly 780 hand, foot and mouth disease (HFMD) cases since the start of the year, the provincial Preventive Medicine Centre said yesterday (Tue 10 Jul 2012). It was a sharp increase compared to the same period last year [2011] when only 12 cases were reported. Some 51 out of every 100 000 people in the province have now contracted the disease, patients being mainly children under 3 years of age.

More than 310 children with mumps have also been reported, nearly double the number in the same period last year [2011].

Update on Illness in Cambodian Children

The World Health Organization (WHO) and the Cambodian Ministry of Health (MOH) released an update on Sunday, July 8, on the active investigation of the undiagnosed illness affecting children in Cambodia. The MOH identified 59 hospitalized cases, of which 52 had died. The age of affected children ranged from 3 months to 11 years old, with the majority being under 3 years of age. Based on the latest laboratory results, a number of samples tested positive for enterovirus 71 (EV71). EV71 is recognized as a cause of hand, foot, and mouth disease (HFMD).
 HFMD is a common viral illness that causes frequent outbreaks. It usually affects children and is characterized by fever, blister-like sores in the mouth, and a skin rash. In outbreaks of EV71, most children have typical symptoms of HFMD and recover without health complications. However, a small number of people with this disease develop severe complications (e.g., meningitis or encephalitis) requiring hospitalization or causing death.

How is EV71 transmitted? EV71 spreads from person to person by direct contact with the infectious virus that causes this disease. This virus is found in nose and throat secretions (such as saliva, sputum, or nasal mucus), fluid in blisters, and in the stool of infected persons. The virus may also be spread when infected persons touch objects and surfaces that are then touched by others.
Infected persons are most contagious during the first week of the illness. The virus can remain in the body for weeks after a person’s symptoms have gone away. This means that infected people can still pass the infection to others even though they may appear well. Also, some people who are infected and spreading the virus, including most adults, may have no symptoms.
HFMD is not transmitted to or from pets or other animals.

How do you prevent infection? There is no vaccine to protect against the virus that causes HFMD. A person can lower their risk of being infected by:
  • Washing hands often with soap and water, especially after changing diapers and using the toilet.
  • Disinfecting dirty surfaces and soiled items, including toys. First wash the items with soap and water; then disinfect them with a solution of chlorine bleach (made by mixing 1 tablespoon of bleach and 4 cups of water).
  • Avoiding close contact such as kissing, hugging, or sharing eating utensils or cups with people with HFMD.

Treatment played key role in killer Cambodian children's disease


The doctor who first alerted the government to Cambodia’s “mystery illness” remains convinced that incorrect treatment in private clinics has played a critical role in the deaths of 64 children....


“Our investigation found that steroids had been used … and there have been studies of the treatment of patients [with EV71] that if you give steroids, those patients die,” Asgari said.

However, Richner, who is awaiting the results of his own toxicology testing being performed in Australia, said that to blame steroids for the deaths of the children was “nonsense”.

“All of the 62 children who died, were treated in private clinics first,” he pointed out. “I think there are wrong [blood transfusions], wrong treatment causing intoxication.

“Lots of medicines in Cambodia are fake or [faulty], and there is no controls over the private clinics buying these medicines,” Richner said. “We still do not know absolutely the reason that these children have died.”

About 8000 ostriches in the Western Cape are being slaughtered after a fresh outbreak of avian flu.
Infected ostriches were found on a farm near Albertinia and on two farms near Heidelberg two weeks ago, The Cape Times reported on Thursday
All birds found on an infected farm had to be killed as part of the government's "stamping out" policy.
The industry had recently stated it had brought the H5N2 disease in the province under control and was a step closer to having its export status re-instated. Thirteen children have been sickened with severe enteroviral infections this year, a record that follows the pattern seen in the mainland and Cambodia, data shows.
There were eight cases of EV in 2010 and nine last year, the Centre for Health Protection said.
Among the 13 cases, eight are imported, all involving mainland children. Five are local "or not classified," it added.
The imported cases include a five- year-old girl and her two brothers, both seven, who tested positive for enteroviruses - which cause hand, foot mouth disease - but negative for the deadly enterovirus71.
On Tuesday, the Centre for Health Protection announced only that the girl and a 39-day-old baby boy from Shenzhen - admitted to the public Prince of Wales Hospital - were confirmed with severe enterovirus infections.

 

Yesterday, the center said the girl and her two brothers, who live in Guangxi, have had fever, headache and neck stiffness since July 1 before traveling to Hong Kong for treatment at Adventist Hospital on July 4.
"Their clinical diagnoses were meningitis due to EV infection and the three children are now in stable condition," the center said.
The baby boy was infected with the Coxsackie virus, which causes a mild form of hand, foot and mouth disease. The baby had a fever for about 10 days.
EV71 has killed 52 of 59 children during the past three months in Cambodia.
Secretary for Food and Health Ko Wing-man said: "In light of all these outbreaks, including the recent detection of avian influenza H5N1 in Bird Street, I would like to remind the citizens to remain vigilant in terms of food and personal hygiene.
"If we are traveling outside Hong Kong, special precautions must be taken as far as the food that we are going to eat as well as our personal hygiene. People who have returned to Hong Kong with symptoms, particularly fever, should seek medical attention."
So far this year, two severe cases of EV71 infection have been reported out of 39 laboratory-confirmed EV71 cases, also a record high.

EV71 in the outbreak of fatal illness in Cambodian children ----------------------------------------------------------- In previous ProMED postings [listed below] the clinical illness in Cambodia was described as occurring mainly in children under 3, with a male/female ratio of 1.3 to 1. It was noted in 14 south and central provinces of the country. Around 60 children were admitted to Kantha Bopha hospital after having been treated in private clinics and most of these died within 24 hours of a syndrome with encephalitis and pulmonary edema, while platelets, liver function, and renal function appeared normal.
Epidemiology and clinical features ---------------------------------- EV71 has been associated with outbreaks of fatal encephalitis in infants in the 1970s in Hungary and Bulgaria (1, 2) and since 1998 with large outbreaks of hand-foot-mouth disease (HFMD) in Southeast Asia (3). A minority of patients in these outbreaks may develop rhombencephalitis and (neurogenic) autonomic dysregulation leading to pulmonary edema, which can be fatal within 24 hours, while laboratory markers of liver and kidney function usually stay within normal range.
The age distribution and male to female ratio of the children in Kantha Bopha is very similar to what was prospectively observed in Children's Hospital 1 in Ho Chi Minh City, Viet Nam among 4000 children hospitalized with HFMD with a median age of 20 months and a male to female ratio of 1.6 to 1. It was also noted that if children progressed to more severe disease this was within a median of 24 hours of admission.
 The case fatality rate was 0.2 percent of all hospitalizations and 6/19 patients with pulmonary edema (TH Khanh, unpublished data). Myoclonus, particularly of the fingers is often the 1st symptom of central nervous system involvement and is seen in a majority of patients with severe disease (4). Deterioration is hallmarked by autonomic dysregulation, which presents as hypertension and tachycardia in addition to uncontrollable fever and may lead to pulmonary edema and death (3). In Taiwan and Malaysia it was noted that a large fraction (one third to half) of the patients progressing to this severe form of disease do not have a rash (5, 6). On the paediatric ICU of the Hospital for Tropical Diseases in Ho Chi Minh City, Viet Nam among the very severe cases 12/47 had no rash and 28/47 had myoclonic jerks. There is no mention of rash, myoclonus, and hypertension in the children in Cambodia, but this would be very interesting to know.
The laboratory findings from Institute Pasteur and the clinical and epidemiological features are compatible with the severe end of the spectrum of disease caused by EV71. The high case fatality rate is striking, but may be due to an underestimation of the total number of cases because less severe cases are not hospitalized and not reported. The absence of the typical rash of HFMD should not rule EV71 out as a causative agent.

Some notes on treatment ----------------------- Guidelines from the Vietnamese ministry of health advice to suppress myoclonus with oral or intravenous phenobarbital, and when suppression is incomplete to instill IVIg at 2g/kg in 2 dosages. Autonomic dysregulation, particularly hypertension, is treated with milrinone (7). Pleconaril, a drug with anti-enteroviral activity, has no activity against EV71 in vitro (11). Evidence for the use of IVIg comes from treatment of enteroviral meningitis in neonates and agammaglobulinaemic patients, but there is only observational and retrospective case data on its efficacy in HFMD and no evidence for who benefits (8-10). Large randomised controlled trials of all treatment options (IVIg, milrinone, magnesium, fluid regimens, etc) are urgently needed for EV71 HFMD. http://www.promedmail.org/

He said Cambodian authorities reportedly were unable to get it under control and had called on the World Health Organisation’s help.



Schools were told to keep clean and health services to report any suspicious cases to the surveillance and rapid response team (SRRT) and to the provincial health office immediately.Following news of a dangerous strain of HFMD killing 64 people and leaving 66 children sick, Cambodian vendors at the Rong Kleu border market yesterday took their schoolchildren to the Thai side. Thai border authorities conducted a random health check on the children and handed out leaflets about HFMD.

The hand, foot and mouth disease (HFMD) scare has resulted in a mass exodus of Cambodian parents with young children into Sa Kaeo province.

The Aranyaprathet immigration checkpoint was yesterday crowded with Cambodian travellers, most of them with young children, eager to enter Thailand.

The Cambodian parents were mainly workers and vendors at the Rong Kleau border market in Thailand.

Thai disease control officials used a thermal scanner to screen for children with a high fever _ one sign of possible HFMD infection.

One child was stopped and tested but the fever was not overly high.

Won Yom, 32, a Cambodian vendor in Poi Pet market in Cambodia, said the Cambodian public health authorities had announced on radio yesterday that the government needed urgent help from the World Health Organisation to control the spread of the virus that has already killed 64 children. That had added to mounting fears among parents with young children and explained why many wanted to come to Thailand.

A Cambodian mother working in Surin province on Tuesday took her two-year-old boy who had a high fever and was suspected of having HFMD to a local hospital.

The incident triggered panic at Sikhoraphum district hospital, which was crowded with children who were mostly there being treated for dengue fever.

Zalo Ann, 30, the Cambodian mother, said she had learned about the spread of HFMD in her home country from the media and when her son became ill, she feared it could be the same disease.

Upon learning about the Cambodian boy and reports of a number of deaths of children in Cambodia from HFMD, parents whose children were being treated at the hospital panicked.
