Case Definitions for Meningitis and Septic Arthritis

A person who received an injection with methylprednisolone acetate produced by the New England Compounding Center (NECC) who has developed any of the following:
  1. Fungal meningitis or non-bacterial and non-viral meningitisa of sub-acute onset, following epidural injection on and after May 21, 2012.
  2. Basilar stroke following epidural injection after May 21, 2012b, who has not received a diagnostic lumbar puncture.
  3. Evidence of spinal osteomyelitis or epidural abscess at the site of injection following epidural or sacroiliac injection after May 21, 2012.
  4. Septic arthritisc or osteomyelitis of a peripheral joint (e.g., knee) diagnosed following joint injection after May 21, 2012.
a Clinically diagnosed meningitis meaning one or more of the following symptoms: headache, fever, stiff neck, or photophobia and a cerebrospinal fluid (CSF) profile showing pleocytosis (>5 white blood cells, adjusting for presence of red blood cells) regardless of glucose or protein levels.
b These people, if possible, should have a lumbar puncture.
c Clinically diagnosed septic arthritis meaning new or worsening pain with presence of effusion or new or worsening effusion.

Suspect Case:

At least 119 sickened


Florida recorded its first death connected to the outbreak of a rare, noncontagious form of meningitis, raising the toll Tuesday to a total of 12 fatalities nationwide.
Earlier in the day, federal authorities reported 11 deaths and 119 persons becoming ill in the outbreak, the Centers for Disease Control and Prevention announced Tuesday. Those figures were an increase from Monday's count of eight deaths and 105 illnesses.

Florida health officials also reported a total of six cases connected to the outbreak -- all in Marion County.

That figure -- one more than federal officials have so far counted in Florida -- means that a total of 120 persons nationwide have become ill, according to state officials.


The outbreak is linked to contaminated steroid injections, and as many as 13,000 people may have received the medicine between May 21 and September 24, the CDC said.

The death of a 70-year-old Florida man occurred in July, prior to the discovery of the contamination, the state's health department said.

CNN) -- A rare, noncontagious form of meningitis has claimed 11 lives and sickened 119 people, an increase from the eight deaths and 105 illnesses previously reported, the Centers for Disease Control and Prevention said Tuesday.
The outbreak is linked to contaminated steroid injections, and as many as 13,000 people may have received the medicine between May 21 and September 24, the CDC said.
The number of reported cases grew significantly in recent days as federal and state authorities continued to investigate the outbreak.
Tennessee is the hardest-hit state, with 35 infections and four deaths, according to the CDC.

The city’s health officials, already grappling with a spurt in dengue and malaria cases over the past few weeks, have a stupefying problem on hand as the common viral flu has taken a rather aggressive form.
The spectrum of virals and similar infections this season have become a reason to worry as hospitals have reported that patients afflicted with the illnesses were showing severe symptoms and longer recovery period.
“The old adage that any fever beyond 4-6 days is usually not a viral has taken a beating. We have now begun to question that theory, considering the disease profile of the patients we are getting,” Dr S Chatterjee, senior consultant in Apollo Hospital’s internal medicine department, said.
The recovery time, doctors said, has gone up to 10 to 12 days and the symptoms also varied.

“We have a range of symptoms from fever and persistent joint pain to symptoms resembling encephalitis, meningitis and hepatitis. We are calling them viral-like diseases because most are showing negative test results for all identified viruses. Even the atypical pathogen profile, a test that diagnoses virals like influenza and H1N1 outside the spectrum of dengue, chikungunya and viral hepatitis, is not showing positive results,” Dr Vikas Ahluwalia of Max Hospital, Saket, said.
In such a situation, doctors put these diseases in the broad category of “viral-like symptoms” - meaning the bug cannot be properly identified.
The symptoms are severe because of the longer recovery period. Doctors said patients, particularly the elderly and diabetics, are showing symptoms characteristic of the H1N1 virus such as acute respiratory distress syndrome (ARDS), in which the lungs get inflamed.
Dr Atul Gogia of Sir Ganga Ram Hospital said: “The viruses mutate over time and these are becoming
resistant to anti-virals just like the growing problem of antibiotic-resistant bacteria. We have patients on ventilator support and have seen deaths from the flu-like symptoms. These patients had tested negative for all notifiable viral diseases such as dengue and malaria.”

Dr Ahluwalia of Max Hospital agreed. Many patients suffering from common viral-like diseases required ventilator support, he said.
“We have seen patients with all kinds of fevers... delirious with fever, aggressive rashes on the body or simply severe, prolonged joint pains characteristic of chikungunya, when the patient has actually tested negative for the bug,” he said.
This has led to a higher number of patients with low or dropping blood platelet counts.
Dr Gogia said there have been cases of two rare viruses, which cause haemoglobin levels to drop. “We have seen cases of the parvovirus, commonly known as parvo, where the haemoglobin level drops drastically. This is usually seen in patients,

The virus could be transferred if an inflicted person sneezed within three feet, he explained. “The coronavirus has no cure yet and little is known about it,” he added.
To avoid the SARS-like disease, Tayag advised: “Wash your hands, limit handshakes and touching other people.

As many as 13,000 people received steroid shots suspected in a U.S. meningitis outbreak, health officials said Monday. But it's not clear how many are in danger.

Officials don't how many of the shots may have been contaminated with meningitis-causing fungus tied to the outbreak. And the figure includes not only those who got them in the back for pain - who are most at risk - but also those who got the shots in other places, like knees and shoulders.

Those injected in joints are not believed to be at risk for fungal meningitis, said Curtis Allen, a spokesman for the Centers for Disease Control and Prevention. Update: Multistate Outbreak of Meningitis and Stroke Associated with Potentially Contaminated Steroid Medication


Summary
The Centers for Disease Control and Prevention (CDC) and the Food and Drug Administration (FDA) continue to work closely with state public health departments on a multistate investigation of fungal meningitis among patients who received an epidural steroid injection. Some of these patients also suffered strokes that may have resulted from their infection. These cases are associated with a potentially contaminated steroid medication prepared by New England Compounding Center (NECC), located in Framingham, Mass. This HAN notice provides updated information about the investigation (including a change in the case definition*), laboratory findings, an expanded voluntary recall of products, and recommendations for clinicians


Background
CDC, in collaboration with FDA, state public health departments, and state boards of pharmacy, has been investigating an ongoing outbreak of meningitis associated with a potentially contaminated steroid medication, preservative-free methylprednisolone acetate (80mg/ml) prepared by New England Compounding Center, located in Framingham, Mass. CDC and state public health departments are actively coordinating outreach to patients who have been exposed to this potentially contaminated medication.
As of October 8, 2012, a total of 105 cases, including 8 deaths, have been reported in 9 states: Florida (4 cases), Indiana (11 cases), Maryland (5 cases, including 1 death), Michigan (21 cases, including 2 deaths), Minnesota (3 cases), North Carolina (2 cases), Ohio (1 case), Tennessee (35 cases, including 4 deaths), and Virginia (23 cases, including 1 death). Fungus has been identified in specimens obtained from at least nine patients, one of whom also had Propionibacterium acnes, of unclear clinical significance, isolated from a post-mortem central nervous system specimen. In addition to an Aspergillus spp. isolated from a Tennessee patient, the fungus Exserohilum rostratum was identified in other patients, indicating the possibility of infections caused by multiple organisms. Fungal meningitis is not transmitted from person to person.
The clinical presentation of infected patients remains consistent with the prior report: onset of symptoms is typically 1 to 4 weeks following injection with a variety of symptoms, including fever, new or worsening headache, nausea, and new neurological deficit (consistent with deep brain stroke). Some of these patients’ symptoms were very mild in nature. Cerebrospinal fluid (CSF) obtained from these patients has typically shown elevated white cell count (with a predominance of neutrophils), low glucose, and elevated protein. As of October 8, no infections resulting from injection into a peripheral joint space have been reported.

Health officials estimate that 13,000 people may have been exposed to contaminated lots of an epidural steroid that has been linked to a rare fungal meningitis that has infected 105 people across nine states, killing eight of them. Fourteen new cases and one new death have been reported since yesterday.

Although the number of cases has increased, the outbreak does not include any new states. The number of people with fungal meningitis, which is not spread person-to-person, has grown by 64 percent since Friday....

Efforts continue MSF and other organizations to tackle Ebola in northeastern Democratic Republic of Congo (DRC). Case management, attention to family and psychosocial support have been a priority for MSF since it launched an emergency intervention in mid-August in the town of Isiro and vicinity.

The count of suspected cases indicates that 34 people have died since the beginning of the Ebola epidemic, according to the Congolese Ministry of Health. In the center of Isiro hospital treatment are currently four people entered. The staff of Doctors Without Borders (MSF) are doing everything in their power to ensure the welfare of these patients.
"Many people have lost their lives, but we are glad we discharged six people after they recover completely," says Alfonso Verdú, MSF emergency coordinator in Isiro. Verdú detailing that since 11 September have confirmed 12 new cases of Ebola, the last one on September 27, so that intervention is far from over.
Together with the Ministry of Health in DRC and other bodies such as the World Health Organization (WHO) and the U.S. Center for Disease Control, MSF is turning in the work to contain the outbreak. The humanitarian organization has increased community awareness activities, essential if the public is well informed about the virus and contribute to the extinction of the outbreak.

Fungal meningitis toll reaches 105; 8 dead


The number of people infected with a rare type of meningitis tied to contaminated steroid injections has climbed to 105, federal health officials said Monday. Eight people have died. 
The drugs were given starting May 21, much earlier than previously suspected, officials with the Centers for Disease Control and Prevention said. Infections and deaths have been reported in nine states. 
Cases have been identified in nine states and health officials fear the numbers will continue to rise as doctors check patients for the symptoms. Doctors and patients alike may not know to look for the unusual infection, which can take weeks to develop after an injection....
