REPUBLIKA.CO.ID, SURABAYA-Chairman of Avian Influenza-Zoonosis Research Center (AIRC)-University of Airlangga (Airlangga University), Surabaya, East Java (East Java), Dr CA Nidom worrying cases of bird flu virus (Avian Influenza / AI) in Sragen, Java East. Because the bird flu makes as much 3,455 ducks or duck is dead and feared to be transmitted to the human body.

Nidom says duck death case due to the fact the AI virus that surfaced today is an accumulation of duck deaths started late last January 2014. However, due to the lack of virus treatment, the mortality rate of up to thousands of ducks each tail.
"In fact, cases of bird flu occurred in 2009 and still not dead. Then appeared again a new virus that is now attacking the ducks," said Nidom told Reuters on Sunday (9/2).

The key, he added, the slower the AI virus handling the more complex case. But she was not sure exactly what type of AI virus that attacks the ducks in Sragen, whether old or new virus virus. But he guessed, 3,455 ducks died suddenly in Sragen the result of marriage or a combination of old and new viruses virus AI.

If so, he mentions the handling of the virus is much more complicated because the virus is a combination virus. "I was worried if the AI virus is left and uncontrolled handling can harm humans because it jumped into the human body," he said.

Moreover, the media increasingly widespread transmission of AI virus. If only the old type of AI virus spread through the air, while the new AI virus can be transmitted through air and water. To solve it, said Nidom, there are several steps.http://translate.google.com/translate?sl=auto&tl=en&js=n&prev=_t&hl=en&ie=UTF-8&u=http%3A%2F%2Fwww.republika.co.id%2Fberita%2Fnasional%2Fumum%2F14%2F02%2F10%2Fn0r5d1-waduh-flu-burung-ancam-warga-di-sragen

Experts say the H7N9 bird flu vaccine clinical evaluation needs at least 6 years

At 9:56 on February 10, 2014Source: First Financial Daily
For the newly developed H7N9 avian influenza DNA vaccine , Professor , Shanghai Institute of Emerging Infectious Diseases Xu Jianqing reproduction yesterday told the " First Financial Daily" reporters , clinical evaluation is divided into three phases , at least six years , there are still some risks during the .
Shanghai Institute of Infectious Diseases , Shanghai Public Health Clinical Center has a new hair and reproduce the 7th news release , H7N9 avian influenza DNA vaccine has completed initial development in Shanghai , has now entered clinical trials reporting stage.
Xu Jianqing told the newspaper, the team developed a vaccine using gene technology. The specific method is to try the main immunogen gene H7N9 is implanted into a mature H7 vaccine vector , equivalent to deposit into the safe and then into human cells , the membrane protein structure of the virus will not be destroyed, can effectively stimulate body to produce protective neutralizing antibodies. 30 mice were vaccinated and then infected with H7N9 avian influenza virus , 30 days without a disease.
However, the effectiveness and safety of vaccines should be verified by further clinical evaluation .
" Phase I clinical evaluation is mainly for vaccine safety , the second phase is the safety and immunogenicity , three are immunogenic and protective ."徐建青said that after the completion of Phase III clinical , be possible to enter the market.
Xu Jianqing said the biggest advantage of DNA vaccine is safe , because the gene vaccine using gene universal vaccine carrier , which has been internationally proven through clinical trials to be safe . " The safety of the vaccine itself , in theory there is no problem , specific to our own vaccine , or pre-clinical and clinical going to go verify ."

However, the main risk is genetic vaccine immunogenicity in humans, monkeys and other larger animals seem weak. " Can be complementary techniques to overcome this weakness on clinical , laboratory we are doing another set of fake virus technology can be used as a complementary technology to enhance the immunogenicity of DNA vaccine ."
In addition , H7N9 viruses are constantly variation, "It depends on which part of the variation , we chose the site of the membrane protein , and the work done from the current data, we have a number of vaccines for 1996 , 2003 and 2007 of H7 viruses have a protective effect . " Xu Jianqing said .
For R & D H7N9 avian influenza virus vaccine , a number of companies and organizations inside and outside there are many breakthroughs . For example in Taiwan Guoguang Biotechnology Co., Ltd. developed the H7N9 bird flu vaccine is expected to enter clinical trials in March ; last May , the U.S. Greffex and Protein Science has also announced the adoption of modified adenovirus vectors through research H7N9 virus vaccine candidates ; addition American Novavax, Canada, Medicago, Sinopharm Group, China biotechnology Co., Ltd. , a subsidiary of Beijing Sinovac Sinovac Biotech Ltd. , Hualan companies, and China Taiwan 's Guoguang biotechnology Co., Ltd. also have declared vaccine development stage , none of these companies are currently entering clinical phase.http://fashion.ifeng.com/health/yiyao/chanjing/detail_2014_02/10/33653020_0.shtml

South Korea culls more birds

Two more cases of AI confirmed

2014-02-09 19:49
Two more cases of bird flu were confirmed on Saturday despite quarantine efforts, raising further concerns over the virus spreading, officials said.

According to the Ministry of Agriculture, Food and Rural Affairs, more than 10,000 fowl at two farms, in Yeongam-gun, South Jeolla Province, and Hwaseong, Gyeonggi Province, were found to have been infected with the highly pathogenic strain of H5N8.

The virus suspicions were reported last Thursday from the two farms after a large number of poultry was found dead.

The quarantine authorities immediately culled more than 150,000 fowl being raised in a 500-meter radius of the concerned sites. Additional culling of more than 100,000 fowl within a 3-kilometer radius is underway, officials said.

As of Sunday afternoon, more than 100 suspected cases have been identified in the country since the first outbreak on Jan. 16, with 51 cases confirmed.

More than 3 million poultry have been slaughtered since then.

Meanwhile, North Korea is also stepping up efforts to defend against the spread of the virus amid concerns over the bird flu, the North Korean media said. According to the North’s state-run Rodong Sinmun newspaper on Saturday, the government is taking preventive measures.

“The quarantine department has drafted a meticulous plan to protect the state from infectious diseases such as bird flu,” it said.

“An emergency meeting was held and revolutionary measures are being taken to prevent the virus spreading through raising local government’s responsibility and role.”

Last week, the North’s official Korean Central News Agency also reported that the state is monitoring the movement of migratory birds and conducting tests on farm visitors and poultry as part of efforts to stave off the outbreak.

The media, however, has not confirmed whether avian influenza has broken out in the isolated state.

According to the World Health Organization’s report late last month, the bird flu has not become a threat for North Korea yet.

Observers said the virus’ spread in neighboring countries such as South Korea and China led North Korea to step up its preventive efforts.  http://www.koreaherald.com/view.php?ud=20140209000255

Effect of closure of live poultry markets on poultry-to-person transmission of avian influenza A H7N9 virus: an ecological study

Effect of closure of live poultry markets on poultry-to-person transmission of avian influenza A H7N9 virus: an ecological study

Summary

Background

Transmission of the novel avian influenza A H7N9 virus seems to be predominantly between poultry and people. In the major Chinese cities of Shanghai, Hangzhou, Huzhou, and Nanjing—where most human cases of infection have occurred—live poultry markets (LPMs) were closed in April, 2013, soon after the initial outbreak, as a precautionary public health measure. Our objective was to quantify the effect of LPM closure in these cities on poultry-to-person transmission of avian influenza A H7N9 virus.

Methods

We obtained information about every laboratory-confirmed human case of avian influenza A H7N9 virus infection reported in the four cities by June 7, 2013, from a database built by the Chinese Center for Disease Control and Prevention. We used data for age, sex, location, residence type (rural or urban area), and dates of illness onset. We obtained information about LPMs from official sources. We constructed a statistical model to explain the patterns in incidence of cases reported in each city on the basis of the assumption of a constant force of infection before LPM closure, and a different constant force of infection after closure. We fitted the model with Markov chain Monte Carlo methods.

Findings

85 human cases of avian influenza A H7N9 virus infection were reported in Shanghai, Hangzhou, Huzhou, and Nanjing by June 7, 2013, of which 60 were included in our main analysis. Closure of LPMs reduced the mean daily number of infections by 99% (95% credibility interval 93—100%) in Shanghai, by 99% (92—100%) in Hangzhou, by 97% (68—100%) in Huzhou, and by 97% (81—100%) in Nanjing. Because LPMs were the predominant source of exposure to avian influenza A H7N9 virus for confirmed cases in these cities, we estimated that the mean incubation period was 3·3 days (1·4—5·7).

Interpretation

LPM closures were effective in the control of human risk of avian influenza A H7N9 virus infection in the spring of 2013. In the short term, LPM closure should be rapidly implemented in areas where the virus is identified in live poultry or people. In the long term, evidence-based discussions and deliberations about the role of market rest days and central slaughtering of all live poultry should be renewed.

How to make sense of H10N8, H1N1, H7N9, and other bewildering bird flu names

H10N8 is the latest bird flu to come out of China and, potentially, decimate humanity like in that Gwyneth Paltrow movie. You may have previously been mortally frightened by news about H1N1, which killed up to 100 million people in 1918 and another 15,000 in 2009 and 2010. Or maybe you’ve been reading about H7N9, which has been connected to just about 100 human deaths and is the cause of a massive poultry cull in Hong Kong this week:
+
A health officer packs dead chickens at a wholesale poultry market in Hong Kong. Reuters/Tyrone Siu
Is H10N8 10 times more deadly than H1N1—or is it eight times, or 80? Not at all. The names refer to the two kinds of spiky protein that influenza A viruses wear on their surfaces, all the better to burrow into the hosts’ cells. H is for hemagglutinin and there are 16 varieties; N is for neuraminidase and there are nine different kinds. All told there are 144 different types of influenza A, from H1N1 to H16N9, all of which are found in birds and some of which are also found in horses, pigs, and humans.
+
Information is beautiful
So why do new versions keep popping up to infect humans? That’s where promiscuous virus behavior comes in. The influenza A virus is fond of a little trick known as reassortment, which means that two viruses infecting the same cell can swap genetic components such as hemagglutinin and neuraminidase proteins. So a chicken in China (virtually all new influenza strains arise in southeast and eastern Asia because of the close contact between humans and livestock) that has two known flu types can give rise to a novel third strain.
+
CDC
To complicate matters, a virus can mix and match any of its eight genes—not just the H and N proteins—and can incorporate genetic material from swine, avian, and human flu strains.
+
virology.ws
In the case of H10N8, only one person has died and another has become sick, making it much too early to issue any apocalyptic warnings. But Chinese and global health authorities are carefully monitoring the virus—especially any indications that it is transmittable between humans, which is when the alarm bells really start going off.http://qz.com/174499/h10n8-bird-avian-flu/

Gaungdong H7N9 Export With Guangxi Onward Transmission

I am inclined to believe this.. The pace is not slowing, and I see funny stuff

February 6, 2014
The Guangxi health bureau said the boy, aged five and from Hengxian county, Nanning, started to show symptoms on Monday and was diagnosed with having the disease on Wednesday.

Health officials are still determining how the boy became infected but said he had been in close contact with his mother, 41.

She began to feel unwell late last month after visiting Zhongshan in Guangdong province. She returned to Nanning on January 28.

The boy remained in a stable condition but his mother’s condition was critical.

The above comments describe an H7N9 familial cluster in Hengxian county, Guangxi, which represents onward transmission of H7N9 bird flu exported from Zhongshan, Guangdong.  Although the Guangxi MoH did note that the mother developed symptoms while in Zhongshan, the report on H7N9 confirmation in her son failed to note that the relationship between the two PCR confirmed cases.  Although WHO has not reported either case, they did describe a similar family cluster in Zhongshan (father, 37M and daughter, 2F in Sanjiaozhen), and failed to note that the confirmed child was the daughter of a confirmed case.

The Guangxi cluster represents the first reported example of H7N9 export coupled with onward transmission, and the export from Zhongshan represents the first example of the H7N9 linkage between an area with one H2H familial cluster (Zhongshan, Guangdong) to another area (Hengxian, Guangxi) with another H2H familial cluster.

These important developments were withheld by the Guangdong MoH, Guangxi MoH, and WHO by the concealing of the relationships in the two familial clusters.

The glaring lack of transparency by the agencies reporting H7N9 confirmed cases while hiding H7N9 clusters is cause for many serious concerns.

H7N9: Each case must be informed, science and do not absence [2014-02-06]

Source: Netease news 2014-02-06 08:50

H7N9 briefing every case lead to "talk bird phobia"-style panic, the reason lies in the disclosure of information asymmetry: although normalization notification mechanism, but the corresponding science remained scarce.
Affected by H7N9, Guangdong and other places of live poultry sales slump, the poultry industry suffered heavy losses. Recently, Guangdong and other places poultry industry associations, companies jointly submitted an open letter to all levels of government, the request "to stop the implementation of each notification H7N9 cases, to avoid the outbreak of ultra-dense reports." Guangdong health department responded, "no legal basis to stop notification", but the outbreak information to the media downplayed the beginning: no longer active communication of information to the media to send cases, notification no longer appears in the word "live poultry contact history" and so on... http://translate.googleusercontent.com/translate_c?depth=1&hl=en&ie=UTF8&prev=_t&rurl=translate.google.com&sl=zh-CN&tl=en&u=http://news.yesky.com/hot/150/35977650.shtml&usg=ALkJrhjeEk5AgHU6kp6PSEmaOAgroJmBUQ

H7N9 virus mutates eyeing the young is it?

Thursday, February 06, 2014 June 02

Shanghai doctor died only 31 years old this year, there are eight Zhejiang patients less than 50 years old
H7N9 virus mutates eyeing the young is it?
Experts said there is no evidence of human to human transmission

Profile picture
This year, the country has confirmed cases of human infection with H7N9 avian influenza 110 cases, 20 cases including Zhejiang, Guangdong, Shanghai death. Recently, our new weekly number of cases of human infection confirmed H7N9 bird flu outbreaks have been close to peak levels during the last year. Although the disease control experts noted that during the Spring Festival is not an outbreak of human infection with H7N9 bird flu, but the reporter Statistics found, H7N9 bird flu has been dropping. Only in Zhejiang Province, 26 cases infected in 8 patients younger than 50 years old. The youngest only 20 years old. Shanghai due to H7N9 avian influenza has been reported in the doctor's death, but also only 31 years old.
H7N9 bird flu tentacles young, because H7N9 virus mutates it? Relatively resistant to young people, why encounter the virus, also fall so fast?
Why multiply the young patients
Young families do not lianzhouzhuan overtime
In Zhejiang, for example, this year's bird flu confirmed in 26 patients, the reporter carefully divided about their ages: 20 to 29 years, 2; 30 to 39 years, 4; 40 to 49 years, 2; 50 to 59 years, 8; 60 to 69 years old, 5; 70 to 79 years old, 5 people. If you draw a line at 50 years old, young patients below 8 people; elderly patients above 18 people.
Overall, though, this year's H7N9 bird flu virus, and there is no special "preference" of young people, the proportion of infections of all ages, not particularly noticeable differences. But the young man must not be taken lightly, can not be considered independent of H7N9 avian influenza with their own, or they will be very dangerous.
Zhejiang University Hospital of Infectious Diseases Professor Liang Weifeng reminded that the young man looked at the body strong, but now to the end of the year, people with a lot of overtime, after some "lianzhouzhuan" office workers, fatigue, colds,
It is easy to let the H7N9 avian influenza virus has an opportunity.
So experts recommend: Young cold not carrying, be sure to pay attention to rest, drink plenty of water, so that the body's immune system to restore as soon as possible, so as to fight with the virus. If there is a continuous fever, it is best to go to hospital blood test, if not high leukocyte index, lung inflammation, and have had contact with poultry experience, the best retest H7N9 virus nucleic acid.
H7N9 virus could mutate it?
There is no evidence of human to human transmission, but still wary
Young filed, Shanghai doctors infection cases occurred, and the doctors have not come into contact with poultry, should H7N9 bird flu virus is showing signs of mutation? Reporter put the question thrown Professor Lu Hongzhou, deputy director of Shanghai Public Health Clinical Center.
Professor Lu Hongzhou said to now, there are no cases of extraordinary human transmission of avian influenza H7N9 occurred.
Late last year, Zhejiang Huzhou Wengxu there had been infected with H7N9 avian influenza have been confirmed cases, then, Li Lanjuan academician after consultation with relevant experts believe that between them are not mutually infected, there is no evidence of human to human transmission.
However, Professor Lu Hongzhou alert, H7N9 avian influenza virus from person to person in a limited capacity exists in theory, he repeatedly stressed that this feature has been recognized by the national authority of the experts. "The so-called limited human-to-human transmission capacity, is characterized by the level of gene sequencing of the virus up analysis." Professor Lu said.
It is reported that the gene sequence of H7N9 avian influenza virus has been announced. This is the last one from the Zhejiang University, State Key Laboratory diagnosis of infectious diseases hospital director Li Lanjuan academicians, isolated and uploaded to international genebanks.
Professor Lu Hongzhou told reporters that since the H7N9 bird flu virus since appeared, one of the country's experts are most concerned about is the thing, there is no monitoring of H7N9 virus mutation key nodes occur.
He believes that the new H7N9 virus in the blood
Coagulation fibroin, the 226th amino acid particularly critical amino acids of this site has the typical characteristics of the human influenza virus genes. Professor Lu Hongzhou, once the site of mutation, meaning the virus and human respiratory epithelial cells are easily combined, people may greatly increase the chance of infection, which is the successor of the will of people.
"This change will come when nobody predicted, but vigilance is necessary." Professor Lu Hongzhou said.

According to Voice of China reported that the current virus is still no significant variation occurred. Currently H7N9 is still a sporadic, Zhejiang, Guangdong and other southern provinces relatively high.
China CDC epidemiology professor Zeng Guang, chief expert believes that the virus occurred in areas which have some chance; Feng Zijian, deputy director of the Centre stressed that three terms, H7N9 virus either from epidemiological or virological , are currently found no significant variation.
These three aspects: one is the ability to communicate is not more easily transmitted from birds to humans, is not it easier person to person? This has not changed, with seeing the same situation last year; second is independent of the variation is not more powerful? Lead to more serious after the onset? Severe cases and deaths occur more of? This situation did not appear; third is to see there is no antiviral drug resistance mutation. It still is alkyl resistant class of drugs, the neuraminidase inhibitors of drug sensitive. In other words, three aspects are not concerned about the variability of new changes.  http://translate.google.com/translate?sl=zh-CN&tl=en&js=n&prev=_t&hl=en&ie=UTF-8&u=http%3A%2F%2Fwww.yznews.com.cn%2Fyzsbs%2Fhtml%2F2014-02%2F06%2Fcontent_543415.htm

Cases of New Deadly Bird Flu Surge in China, Experts Say

Cases of the new H7N9 avian influenza in China are surging alarmingly, flu experts warned this week.
There are now about 300 confirmed cases, with more appearing every day. Roughly a quarter of the victims have died.
The first human cases were reported only last March. By contrast, the H5N1 influenza virus, another lethal strain that jumped from birds to people, first appeared in 2003 and took almost five years to reach the 300-case mark.

“H7N9 is blowing right by H5N1,” said Michael T. Osterholm, director of the Center for Infectious Disease Research and Policy at the University of Minnesota. “I’m really worried about that.”
At the same time, an even newer avian flu in China has killed its first human victim. That strain, known as H10N8, has been confirmed in only two people — a 73-year-old woman who died in December but whose infection was confirmed later, and a 55-year-old woman from the same province hospitalized in January. Both had contact with live poultry.
(Influenzas are described by the shapes of two protuberances on their surfaces: the hemagglutinin “spike,” or “H,” that attaches to cells, and the neuraminidase “helicopter,” or “N,” that chops off receptors, allowing new viruses to escape. There are 18 known types of H and 11 of N.)
While the newer influenza virus is worrisome, a mere two cases is “a far, far cry” from the risk posed by H7N9, said Ruben Donis, chief of preparedness for the flu division of the Centers for Disease Control and Prevention. He noted that the H7N9 virus had infected people in 10 Chinese provinces and was close to the borders of Vietnam, Cambodia and other poor Southeast Asian nations less adept than China at flu surveillance.

Birds appear to be spreading the H7N9 virus more through their breath than through feces, the normal infection route, Dr. Osterholm said, which means that fecal-sample testing in live markets may miss cases.
Monitoring the spread of the H7N9 and H10N8 viruses is difficult because neither makes chickens sick. Poultry farmers resist testing because a positive test forces them to destroy flocks that appear healthy. By contrast, the H5N1 virus rapidly kills flocks, so farmers want it extinguished.
Although human-to-human transmission of the H7N9 virus has not been confirmed, there have been clusters of two or three cases within families.
Because some relatives were reported to have developed symptoms days later than others, Henry L. Niman, a flu watcher in Pittsburgh who monitors news reports, said the most likely explanation was that they infected each other.
Dr. Osterholm disagreed, saying family members could have been exposed to the same birds but fallen ill on different days.  http://www.nytimes.com/2014/02/05/health/cases-of-new-deadly-bird-flu-surge-in-china-experts-say.html?_r=0

Human infection with avian influenza A(H7N9) virus – update

Disease outbreak news
On 4 February 2014, the National Health and Family Planning Commission (NHFPC) of China notified WHO of eight additional laboratory-confirmed cases of human infection with avian influenza A(H7N9) virus, including two deaths. .

Details of the cases are as follows

Six of the cases are male. The age range is 4 to 84 years old. Cases were reported from Zhejiang (4), Guangdong (3) and Jiangsu (1). Three cases are currently in a critical condition, two cases are currently in a severe condition and one is currently in a mild condition. All cases are reported to have had a history of exposure to poultry or a live poultry market.
  • A 42 year-old man from Hangzhou City, Zhejiang Province, who became ill on 25 January and was admitted to hospital on 30 January. He is currently in a severe condition. The patient has a history of exposure to poultry.
  • An 84 year-old man from Hangzhou City, Zhejiang Province, who became ill on 24 January and was admitted to hospital on 28 January. He is currently in a critical condition. The patient has a history of exposure to poultry.
  • A 56 year-old man from Jinhua City, Zhejiang Province, who became ill on 24 January and was admitted to hospital on 1 February. He is currently in a severe condition. The patient has a history of exposure to a live poultry market.
  • A 51 year-old man from Shaoxing City, Zhejiang Province, who became ill on 27 January and was admitted on 1 February. He is currently in a critical condition. The patient has history of exposure to a live poultry market.
  • A 4 year-old girl from Zhaoqing City, Guangdong Province, who became ill on 26 January and was admitted to hospital on 3 February. She is currently in a mild condition. The patient has a history of exposure to live poultry.
  • A 76 year-old man from Yangjiang City, Guangdong Province, who became ill on 27 January and was admitted to hospital on 1 February. He died on 3 February. The patient had a history of exposure to live poultry.
  • A 52 year-old man from Huizhou City, Guangdong Province, who became ill on 25 January and was admitted to hospital on 2 February. He died on 3 February. The patient had a history of exposure to live poultry.
  • A 59 year-old woman from Wuxi City, Jiangsu Province, who became ill on 26 January and was admitted to hospital on 29 January. She is currently in a critical condition. The patient has history of exposure to a live poultry market.
So far, there is no evidence of sustained human-to-human transmission.
The Chinese Government continues to take the following surveillance and control measures:
  • strengthen surveillance and situation analysis;
  • reinforce case management and treatment;
  • conduct risk communication with the public and release information;
  • strengthen international collaboration and communication; and
  • conduct scientific studies.

Sporadic human cases

While the recent report of avian influenza A(H7N9) virus being detected in live poultry imported from the mainland to Hong Kong SAR, shows the potential for the virus to spread through live poultry, at this time there is no indication that international spread of avian influenza A(H7N9) has occurred through humans or animals.
Further sporadic human cases of A(H7N9) infection are expected in affected and possibly neighbouring areas, especially given expected increases in the trade and transport of poultry associated with the Lunar New Year.

WHO advice

WHO advises that travellers to countries with known outbreaks of avian influenza should avoid poultry farms, or contact with animals in live bird markets, or entering areas where poultry may be slaughtered, or contact with any surfaces that appear to be contaminated with faeces from poultry or other animals. Travellers should also wash their hands often with soap and water. Travellers should follow good food safety and good food hygiene practices.
WHO does not advise special screening at points of entry with regard to this event, nor does it currently recommend any travel or trade restrictions.
As always, a diagnosis of infection with an avian influenza virus should be considered in individuals who develop severe acute respiratory symptoms while travelling or soon after returning from an area where avian influenza is a concern.
WHO encourages countries to continue strengthening influenza surveillance, including surveillance for severe acute respiratory infections (SARI) and to carefully review any unusual patterns, in order to ensure reporting of human infections under the IHR (2005), and continue national health preparedness actions.
