[ProMED-mail would like to thank Dr Memish for sharing this information on the 3 new cases associated with the cluster of severe acute respiratory illness (SARI) cases in Alhasa, Eastern Province.

As enumerated in the above report, there are now 13 cases associated with this cluster, 7 of whom have died (54 per cent). As with the 10 previously reported cases, all 3 of these have existing comorbidities. The statement from the above report -- "So far there is no apparent community transmission and transmission seem linked to one HCF [health care facility]" supports a conclusion that this outbreak is a nosocomial outbreak, and the reason that all confirmed cases to date have had existing comorbidities is that they were presumably infected with the nCoV while hospitalized in this HCF for other pre-existing illnesses.

The dates of onset of the 13 cases in the cluster in Alhasa are:
1 case on 14 Apr 2017;
1 case on 15 Apr 2013;
2 cases on 17 Apr 2017,
1 case on 19 Apr 2013;
4 cases on 22 Apr 2013;
1 case on 27 Apr 2013;
1 case on 28 Apr 2013;
1 case on 30 Apr 2013;
1 case on 1 May 2013.

As for route of transmission within the hospital, we await findings of epidemiologic investigations currently under way.

As of today, 5 May 2013, the breakdown of total confirmed cases of SARI associated with infection with the nCoV (and deaths) by country of report is:
- Saudi Arabia: 22 (13 deaths)
- Jordan: 2 (2 deaths)
- UK: 4 (one patient from Qatar -- under treatment, 3 patients from UK -- one with history of travel to Saudi Arabia and Pakistan prior to illness; one recovered, 2 deaths)
- Germany: 2 (one patient from Qatar -- discharged, one patient from the UAE -- one death).

Including the 3 newly confirmed cases, there are now 30 confirmed cases with 18 deaths (60 per cent CFR).

New SARS-related virus kills five more in Saudi Arabia

Saudi Arabia has revealed seven new cases of a novel coronavirus, including five deaths — a surprise announcement that is raising transparency concerns and seems to have caught even the World Health Organization off-guard.
The novel coronavirus, which is genetically related to the virus that causes SARS, has been confirmed in 24 people, 16 of whom died. Prior to these new cases, the last confirmed infection was reported in March.
The seven patients have been known to Saudi Arabian officials for a few days now, the kingdom’s deputy minister for public health, Dr. Ziad Memish, told the Toronto Star in a phone interview. However, laboratory tests only confirmed that the seven cases were coronavirus infections on Wednesday, after which a news release was issued and the WHO was notified, Memish said.
“We heard the rumours and that’s why we sent the team to investigate,” he said. “We have been investigating it for the last few days.”
WHO spokesperson Gregory Hartl said the UN health agency was notified late Wednesday and it is standard for governments to wait until they have laboratory confirmation before reporting new cases involving viruses with pandemic potential.
But to Dr. Jody Lanard, a Brooklyn-based risk communication expert who has consulted for the WHO in the past, it appears the Saudi government has been demonstrating a lack of transparency since the new coronavirus first came to public attention in September.
“Having analyzed communication as an expert in so many disease outbreaks, Saudi Arabia is showing all the signs of hiding information (and) delaying reports of information,” Lanard said. “We had the SARS outbreak in 2003 and we saw the repercussions of its widespread reach that partly resulted from delayed information — that’s a lesson that should already be learned.”
Lanard was referring to the Chinese government’s initial coverup of SARS, which ultimately spread around the world and killed roughly 800 people. All eyes have been on China again these days because of a new bird flu outbreak, but public health officials say the Chinese have been much more transparent this time around.
Nobody knows yet how people are getting infected by the new coronavirus, which is genetically similar to viruses found in bats. Most cases have been in Saudi Arabia but infections have also been reported in Jordan and Qatar, as well as some imported cases in Germany and the United Kingdom.
There has been some evidence of limited human-to-human transmission — in England, three family members tested positive for the virus after one travelled to Pakistan and Saudi Arabia and fell sick upon his return.
Hartl said the WHO has received little information about the new cases, including the patients’ genders, ages or when they fell sick. He said the seven patients are not from the same family, nor do they have any recent travel history or exposure to animals.
They were all treated at the same hospital in Al-Ahsa, in eastern Saudi Arabia, Memish said.
WHO’s top infectious disease expert, Dr. Keiji Fukuda told The Canadian Press on Thursday that the WHO wants more and quicker information about new coronavirus cases.
“The more information the better — to help Saudi (Arabia), to help other neighbouring countries, to help the world be better ready,” Hartl said.
But Memish maintains that his government has been as forthcoming as possible when it comes to sharing new information, particularly with the WHO.
“We have been very transparent and we have given all the information on the first few cases,” he said. “So, all we’re asking is that you give us a few days to get the information sorted out and then you will know all the other details.”
But Lanard said far more information about the new coronavirus should be known by now. She said the WHO needs to tread carefully in its interactions with governments and in sharing information publicly — but in this case, the circumstances may warrant a more forceful position, she said.
“I think there are times when they should take off the kid gloves and put on the boxing gloves and let the world know that a country is putting the rest of the world at risk,” she said. “And this is one of those times.” http://www.thestar.com/news/world/2013/05/02/new_sarsrelated_virus_kills_five_more_in_saudi_arabia.html

3 new coronavirus cases discovered in Al-Ahsa

Sunday 5 May 2013
Last Update 5 May 2013 2:44 am
Three more people have been diagnosed with coronavirus, raising the number of victims to 10.
The newly identified victims are also from Al-Ahsa province. It brings to 27 the global total of confirmed cases, including 16 deaths.
Last week, the virus killed five people in the Kingdom and put two more patients in intensive care in Al-Ahsa.
Health Ministry Spokesman Dr. Khalid Al-Mirghalani said the two patients infected last week remain in ICU, while among the three new cases, only one is in ICU.
Meanwhile, five Saudis who died last week had not traveled abroad, a Health Ministry doctor said yesterday.

Dr. Ziad Memish, who heads the ministry's Disease Prevention Unit, told AFP: “After questioning relatives, it turned out that none of these people had been abroad before being infected.”
The ministry was working closely with the WHO over cases registered in Saudi Arabia but did not need help from foreign medical teams, Memish said.
Memish said the doctors found out that the 10 patients have contracted the disease locally, and that they have no history of recent travel abroad.
He stressed that there was no cause for alarm about the infection as the ministry had already taken all preventive measures to keep the disease under control.  http://www.arabnews.com/news/450501

Looks like some may have gotten sick from the hospital

...They had not travelled abroad, suggesting they contracted the novel coronavirus (NCoV) locally. Saudi Arabia's deputy minister for public health said that all the cases were from a single hospital over the last few days and he had no reason to believe a larger number of people were exposed NCoV.
"All the cases that have been reported were in the same hospital in Alhasaa. We have not found any cases anywhere else in the eastern region," Dr Ziad A Memish told The National.
Dr Memish said that Saudi Arabia's medical system had undertaken surveillance measures since the first report of the virus last summer.
"We are asking hospitals to test all individuals" who present symptoms consistent with the virus, he said.
"We are recommending precautionary measures such as covering one's mouth and nose when sneezing, maintaining good hygiene. Hopefully this will prevent the spread until we have more information."...     http://www.counselheal.com/articles/...are-report.htm

Victims of SARS-like virus didn’t travel

Victims of SARS-like virus didn’t travel

RIYADH – Five Saudis who died after contracting a new SARS-like virus last week had not traveled abroad, a health ministry official said on Saturday.

“After questioning relatives, it turned out that none of these people had been abroad before being infected,” Dr. Ziad Mimish, Undersecretary of Ministry of Health for Public Health, told AFP.

The outbreak occurred in Al-Ahsaa, which is near Bahrain and Qatar.

The original seven cases in this group all reportedly had “multiple comorbidities.” A comorbidity is medical-speak for a disease or condition. The ages of the initial seven cases ranged from 24 to 94, though most were in their 50s.

The ministry was working closely with the World Health Organization (WHO) over cases registered in Saudi Arabia but did not need help from foreign medical teams, Mimish said.

A system was put in place across the Kingdom to detect suspected cases and to monitor the condition of medics in contact with the patients, he added.

The health ministry said it is taking “all precautionary measures for persons who have been in contact with the infected people... and has taken samples from them to examine if they are infected.”

The ministry has started conducting surveillance search of all persons who mingled with the patients, whether family members or acquaintances, and testing their samples, to determine if any of them has any sign of contracting the virus.

The ministry said that its announcement comes as part of its transparency policy toward public regarding all the health matters that might affect them.

The WHO reported on Friday that three new cases of the virus were detected in Saudi Arabia, bringing to 27 the global total of confirmed cases, including 16 deaths.

The mysterious virus has been deadliest in Saudi Arabia, and the other cases were reported in Jordan, Germany and Britain.

In Geneva, a spokesman for the World Health Organization would not comment on why so many of the cases have occurred in the Kingdom.

“We have received a formal notification from the Saudis about these cases,” spokesman Glenn Thomas told AFP.

When asked if the WHO is planning to issue a travel warning for Saudi Arabia, he said this was “unlikely” since “there has been no change in the risk assessment.”

Coronaviruse causes most common colds and pneumonia, but are also to blame for unusual conditions such as SARS which killed more than 800 people when it swept out of China in 2003.

The new virus is different from SARS, in that it can cause rapid kidney failure.

Known as novel coronavirus or hCoV-EMC, the virus was first detected in mid-2012 and is a cousin of Severe Acute Respiratory Syndrome (SARS) virus, which triggered a scare 10 years ago when it erupted in East Asia, leaping to humans from animal hosts.

Novel coronavirus infection - update

03/05/2013

The Ministry of Health in Saudi Arabia has informed WHO of an additional three laboratory confirmed cases of infection with the novel coronavirus (nCoV).

The three patients are from the same location as reported in the Disease Outbreak News published on 2 May 2013. They are currently in critical condition.

The government is conducting ongoing investigation into this outbreak.

According to preliminary information, at least two confirmed cases are from the same family.

From September 2012 to date, WHO has been informed of a global total of 27 laboratory confirmed cases of human infection with nCoV, including 16 deaths.

Based on the current situation and available information, WHO encourages all Member States (MS) to continue their surveillance for severe acute respiratory infections (SARI) and to carefully review any unusual patterns. WHO is currently working with international experts and countries where cases have been reported to assess the situation and review recommendations for surveillance and monitoring.

All MS are reminded to promptly assess and notify WHO of any new case of infection with nCoV, along with information about potential exposures that may have resulted in infection and a description of the clinical course.

WHO does not advise special screening at points of entry with regard to this event nor does it recommend that any travel or trade restrictions be applied.

WHO continues to closely monitor the situation.
