Cases of New Deadly Bird Flu Surge in China, Experts Say

Cases of the new H7N9 avian influenza in China are surging alarmingly, flu experts warned this week.
There are now about 300 confirmed cases, with more appearing every day. Roughly a quarter of the victims have died.
The first human cases were reported only last March. By contrast, the H5N1 influenza virus, another lethal strain that jumped from birds to people, first appeared in 2003 and took almost five years to reach the 300-case mark.

“H7N9 is blowing right by H5N1,” said Michael T. Osterholm, director of the Center for Infectious Disease Research and Policy at the University of Minnesota. “I’m really worried about that.”
At the same time, an even newer avian flu in China has killed its first human victim. That strain, known as H10N8, has been confirmed in only two people — a 73-year-old woman who died in December but whose infection was confirmed later, and a 55-year-old woman from the same province hospitalized in January. Both had contact with live poultry.
(Influenzas are described by the shapes of two protuberances on their surfaces: the hemagglutinin “spike,” or “H,” that attaches to cells, and the neuraminidase “helicopter,” or “N,” that chops off receptors, allowing new viruses to escape. There are 18 known types of H and 11 of N.)
While the newer influenza virus is worrisome, a mere two cases is “a far, far cry” from the risk posed by H7N9, said Ruben Donis, chief of preparedness for the flu division of the Centers for Disease Control and Prevention. He noted that the H7N9 virus had infected people in 10 Chinese provinces and was close to the borders of Vietnam, Cambodia and other poor Southeast Asian nations less adept than China at flu surveillance.

Birds appear to be spreading the H7N9 virus more through their breath than through feces, the normal infection route, Dr. Osterholm said, which means that fecal-sample testing in live markets may miss cases.
Monitoring the spread of the H7N9 and H10N8 viruses is difficult because neither makes chickens sick. Poultry farmers resist testing because a positive test forces them to destroy flocks that appear healthy. By contrast, the H5N1 virus rapidly kills flocks, so farmers want it extinguished.
Although human-to-human transmission of the H7N9 virus has not been confirmed, there have been clusters of two or three cases within families.
Because some relatives were reported to have developed symptoms days later than others, Henry L. Niman, a flu watcher in Pittsburgh who monitors news reports, said the most likely explanation was that they infected each other.

On 4 February 2014, the National Health and Family Planning Commission (NHFPC) of China notified WHO of eight additional laboratory-confirmed cases of human infection with avian influenza A(H7N9) virus, including two deaths. .

Details of the cases are as follows

Six of the cases are male. The age range is 4 to 84 years old. Cases were reported from Zhejiang (4), Guangdong (3) and Jiangsu (1). Three cases are currently in a critical condition, two cases are currently in a severe condition and one is currently in a mild condition. All cases are reported to have had a history of exposure to poultry or a live poultry market.
  • A 42 year-old man from Hangzhou City, Zhejiang Province, who became ill on 25 January and was admitted to hospital on 30 January. He is currently in a severe condition. The patient has a history of exposure to poultry.
  • An 84 year-old man from Hangzhou City, Zhejiang Province, who became ill on 24 January and was admitted to hospital on 28 January. He is currently in a critical condition. The patient has a history of exposure to poultry.
  • A 56 year-old man from Jinhua City, Zhejiang Province, who became ill on 24 January and was admitted to hospital on 1 February. He is currently in a severe condition. The patient has a history of exposure to a live poultry market.
  • A 51 year-old man from Shaoxing City, Zhejiang Province, who became ill on 27 January and was admitted on 1 February. He is currently in a critical condition. The patient has history of exposure to a live poultry market.
  • A 4 year-old girl from Zhaoqing City, Guangdong Province, who became ill on 26 January and was admitted to hospital on 3 February. She is currently in a mild condition. The patient has a history of exposure to live poultry.
  • A 76 year-old man from Yangjiang City, Guangdong Province, who became ill on 27 January and was admitted to hospital on 1 February. He died on 3 February. The patient had a history of exposure to live poultry.
  • A 52 year-old man from Huizhou City, Guangdong Province, who became ill on 25 January and was admitted to hospital on 2 February. He died on 3 February. The patient had a history of exposure to live poultry.
  • A 59 year-old woman from Wuxi City, Jiangsu Province, who became ill on 26 January and was admitted to hospital on 29 January. She is currently in a critical condition. The patient has history of exposure to a live poultry market.
So far, there is no evidence of sustained human-to-human transmission.
The Chinese Government continues to take the following surveillance and control measures:
  • strengthen surveillance and situation analysis;
  • reinforce case management and treatment;
  • conduct risk communication with the public and release information;
  • strengthen international collaboration and communication; and
  • conduct scientific studies.

Sporadic human cases

While the recent report of avian influenza A(H7N9) virus being detected in live poultry imported from the mainland to Hong Kong SAR, shows the potential for the virus to spread through live poultry, at this time there is no indication that international spread of avian influenza A(H7N9) has occurred through humans or animals.
Further sporadic human cases of A(H7N9) infection are expected in affected and possibly neighbouring areas, especially given expected increases in the trade and transport of poultry associated with the Lunar New Year.

WHO advice

WHO advises that travellers to countries with known outbreaks of avian influenza should avoid poultry farms, or contact with animals in live bird markets, or entering areas where poultry may be slaughtered, or contact with any surfaces that appear to be contaminated with faeces from poultry or other animals. Travellers should also wash their hands often with soap and water. Travellers should follow good food safety and good food hygiene practices.
WHO does not advise special screening at points of entry with regard to this event, nor does it currently recommend any travel or trade restrictions.
As always, a diagnosis of infection with an avian influenza virus should be considered in individuals who develop severe acute respiratory symptoms while travelling or soon after returning from an area where avian influenza is a concern.
WHO encourages countries to continue strengthening influenza surveillance, including surveillance for severe acute respiratory infections (SARI) and to carefully review any unusual patterns, in order to ensure reporting of human infections under the IHR (2005), and continue national health preparedness actions.

Guangxi boy confirmed with H7N9 virus after mother contracts bird flu

..Health authorities in Guangxi province confirmed a boy had contracted the H7N9 strain of bird flu, a day after his mother was confirmed to have the virus.
The Guangxi health bureau said the boy, aged five and from Hengxian county, Nanning, started to show symptoms on Monday and was diagnosed with having the disease on Wednesday.
Health officials are still determining how the boy became infected but said he had been in close contact with his mother, 41.
She began to feel unwell late last month after visiting Zhongshan in Guangdong province. She returned to Nanning on January 28.
The boy remained in a stable condition but his mother’s condition was critical.

A report published in the journal Clinical Infectious Diseases by Dr David Hui of the Chinese University of Hong Kong said H7N9 flu virus spreads easily and more among the Chinese because of the IFITM3 gene (interferon-induced transmembrane protein-3 gene).

The IFITM3 gene is the one that increases influenza disease severity. It is more predominant among the Chinese people. Caucasians hardly have them, Mr Hui said.
This seemed to confirm an earlier study that said one factor the Chinese are susceptible to the H7N9 flu virus was because they have lower levels of an immune protective mechanism known as CD8+ T lymphocyte (CTL) immunity.
Dr Hui said that 70 per cent of Chinese patients who had experienced severe influenza infection had the IFITM3 susceptibility gene, which could explain the emergence or presence of infection clusters among family members.
"Because the risk genotype occurs with such a high frequency [in Chinese people], its effect translates to a large population-attributable risk of 54.3% for severe infection in the Chinese population studied compared with 5.4% in Northern Europeans," Dr Hui said.
On Tuesday, China reported two new deaths due to the H7N9 flu virus. The death tally has now reached a total of 25 since January, while the number of human infections has reached 113 cases. Zhejiang and Guangdong were the areas most affected.

CAMBODIA-First H5N1 case of 2014 reported

"This relation should be studied further to understand the mechanism of how the internal genes of H9N2 virus transfer to viruses that infect people, and how the avian influenza virus transfers between species."
The authors say the findings suggest that JX346 might have originated from multiple reassortments between different avian flu viruses.

Senior author Yuelong Shu, PhD, of the Chinese Center for Disease Control and Prevention commented in a Lancet press release, "The H10 and H8 gene segments might have derived from different wild bird influenza viruses [that] reassorted to give rise to a hypothetical H10N8 virus in wild birds, which infected poultry and then reassorted with H9N2 viruses in poultry to give rise to the novel reassortant JX346 (H10N8) virus."
In other observations, the researchers say the patient's chronic medical conditions could have played a role in her death. But they found that the novel virus was "overwhelmingly dominant" in her tracheal specimens, which indicates that it was involved in her demise.

Clinical and epidemiological characteristics of a fatal case of avian influenza A H10N8 virus infection: a descriptive study

Background

Human infections with different avian influenza viruses—eg, H5N1, H9N2, and H7N9—have raised concerns about pandemic potential worldwide. We report the first human infection with a novel reassortant avian influenza A H10N8 virus.

Methods

We obtained and analysed clinical, epidemiological, and virological data from a patient from Nanchang City, China. Tracheal aspirate specimens were tested for influenza virus and other possible pathogens by RT-PCR, viral culture, and sequence analyses. A maximum likelihood phylogenetic tree was constructed.

Findings

A woman aged 73 years presented with fever and was admitted to hospital on Nov 30, 2013. She developed multiple organ failure and died 9 days after illness onset. 
 
 A novel reassortant avian influenza A H10N8 virus was isolated from the tracheal aspirate specimen obtained from the patient 7 days after onset of illness.
 
Sequence analyses revealed that all the genes of the virus were of avian origin, with six internal genes from avian influenza A H9N2 viruses. The aminoacid motif GlnSerGly at residues 226—228 of the haemagglutinin protein indicated avian-like receptor binding preference. A mixture of glutamic acid and lysine at residue 627 in PB2 protein—which is associated with mammalian adaptation—was detected in the original tracheal aspirate samples. 
 
The virus was sensitive to neuraminidase inhibitors. Sputum and blood cultures and deep sequencing analysis indicated no co-infection with bacteria or fungi. Epidemiological investigation established that the patient had visited a live poultry market 4 days before illness onset.

Interpretation

The novel reassortant H10N8 virus obtained is distinct from previously reported H10N8 viruses. The virus caused human infection and could have been associated with the death of a patient.

  • A 63 year-old man from Shenzhen City, Guangdong Province, who became ill on 27 January and was admitted to hospital on 1 February and died on the same day. The patient had a history of exposure to a live poultry market.
So far, there is no evidence of sustained human-to-human transmission.
The Chinese Government continues to take the following surveillance and control measures:
  • strengthen surveillance and situation analysis;
  • reinforce case management and treatment;
  • conduct risk communication with the public and release information;
  • strengthen international collaboration and communication; and
  • conduct scientific studies.

Sporadic human cases

While the recent report of avian influenza A(H7N9) virus being detected in live poultry imported from the mainland to Hong Kong SAR, shows the potential for the virus to spread through live poultry, at this time there is no indication that international spread of avian influenza A(H7N9) has occurred through humans or animals.
Further sporadic human cases of A(H7N9) infection are expected in affected and possibly neighbouring areas, especially given expected increases in the trade and transport of poultry associated with the Lunar New Year.

WHO advice

WHO advises that travellers to countries with known outbreaks of avian influenza should avoid poultry farms, or contact with animals in live bird markets, or entering areas where poultry may be slaughtered, or contact with any surfaces that appear to be contaminated with faeces from poultry or other animals. Travellers should also wash their hands often with soap and water. Travellers should follow good food safety and good food hygiene practices.
WHO does not advise special screening at points of entry with regard to this event, nor does it currently recommend any travel or trade restrictions.
As always, a diagnosis of infection with an avian influenza virus should be considered in individuals who develop severe acute respiratory symptoms while travelling or soon after returning from an area where avian influenza is a concern.
WHO encourages countries to continue strengthening influenza surveillance, including surveillance for severe acute respiratory infections (SARI) and to carefully review any unusual patterns, in order to ensure reporting of human infections under the IHR (2005), and continue national health preparedness actions.

1st New Human Case of Avian Influenza H5N1 in Cambodia in 2014


The Ministry of Health (MoH) of the Kingdom of Cambodia wishes to advise members of the public that one new human case of avian influenza has been confirmed for the H5N1 virus. This is the 1st case of this year and the 48th people to become infected with the H5N1 virus in Cambodia. Case is from Kampong Thom province and he is currently in a stable condition. Of the 48 confirmed cases, 36 were children under 14, and 27 of the 48 were female. In addition, since the first case happened in Cambodia in 2005 there were only 16 cases survived.

The 1st case this year, a 5-year-old boy from Chongda village, Tbaung Kropeur commune, Santuk district, Kampong Thom province, was confirmed positive for H5N1 human avian influenza on 1st February 2014 by Institut Pasteur du Cambodge. The boy had onset fever and headache on 24th January 2014, his parents sought treatment at a private clinic on 27th January with symptoms of high fever and cough, but his condition worsened. The boy was admitted to Jayavarman VII Hospital, Siem Reap, on 30th January 2014. Tamiflu was administered on 31st January.

Investigations in the village by the Ministry of Health’s Rapid Response Teams (RRT) and the Ministry of Agriculture, Forestry and Fishery’s (MAFF) Animal Health Task Force revealed that around 200 chickens died suddenly mid-January. The boy’s parents and relatives prepared the dead chickens for food while the boy stayed around the area..........
