OIE Reports Avian Influenza A(H7N9) in Zhejiang

CHINA - The Chinese veterinary authorities have reported an outbreak of low pathogenic avian influenza A(H7N9) in Zhejiang.
The World Organisation for Animal Health (OIE) received follow-up report no. 6 on 26 April.
According to the report, three birds were found affected.
Gene sequencing performed at the Harbin Veterinary Research Institute, Chinese Academy of Agricultural Sciences (national reference laboratory for avian influenza) showed that it is low pathogenic avian influenza virus (LPAI).

WHO Reports Six Human H5N1 Avian Influenza Cases During Past Six Weeks

According to a World Health Organization (WHO) summary and assessment Friday, from March 12 to April 26, 2013, there were a total of six new laboratory confirmed human cases with avian influenza A(H5N1) virus infection.

The cases were from the following countries: Bangladesh (1), Cambodia (1), Egypt (2 ) and Vietnam(2). The UN agency says that afterinvestigation, the six cases are considered sporadic; however, more cases are likely in the future.
So far this yearCambodia has reported 10 cases of the lethal virus in humans, resulting in eight fatalities. Health authorities say cases do not seem to be linked directly, and most had contact with sick poultry in their villages.
Since 2003, there have been 628 laboratory-confirmed human cases with avian influenza A(H5N1) virus infection have been officially reported to WHO from 15 countries, of which 374 died (approximately 60 %).
According to the WHO, the H5N1 virus subtype  (different from the H7N9 strain circulating in the current China outbreak)   – a highly pathogenic AI virus- first infected humans in 1997 during a poultry outbreak in Hong Kong SAR, China. Since its widespread re-emergence in 2003 and 2004, this avian virus has spread from Asia to Europe and Africa and has become entrenched in poultry in some countries, resulting in millions of poultry infections, several hundred human cases, and many human deaths.
The case fatality rate for H5N1 virus infections in people is much higher compared to that of seasonal influenza infections.
Clinically, in many patients, the disease caused by the H5N1 virus follows an unusually aggressive clinicalcourse, with rapid deterioration and high fatality. Like most emerging disease, H5N1 influenza in humans is poorly understood.
The incubation period for H5N1 avian influenza may be longer than that for normal seasonal influenza, which is around two to three days. Current data for H5N1 infection indicate an incubation period ranging from two to eight days and possibly as long as 17 days. WHO currently recommends that an incubation period of seven days be used for field investigations and the monitoring of patient contacts.
Initial symptoms include a high fever, usually with a temperature higher than 38oC, and other influenza-like symptoms. Diarrhoea, vomiting, abdominal pain, chest pain, and bleeding from the nose and gums have also been reported as early symptoms in some patients.
One feature seen in many patients is the development of lower respiratory tract early in the illness. Onpresent evidence, difficulty in breathing develops around five days following the first symptoms. Respiratory distress, a hoarse voice, and a crackling sound when inhaling are commonly seen. Sputum production is variable and sometimes bloody. http://www.theglobaldispatch.com/who-reports-six-human-h5n1-avian-influenza-cases-during-past-six-weeks-60184/

Chinese Premier calls for 'high alert' as number of bird flu infections continues to rise

CHINESE Premier Li Keqiang has called on health officials and residents to remain vigilant against the bird flu as the lethal virus continues to spread.

Premier Li Keqiang is urging caution against the deadly new strain of bird flu that has so far killed 24 people.

Mr Li insisted efforts to tackle the virus have so far been effective.

"We need to stay on high alert as this is a new virus, and we don't have all the information," Li said Sunday during a visit to the nation's disease control agency in Beijing, according to a report posted on China National Radio's website.

A leading American virologist has said there is no evidence the deadly H7-N9 bird flu has yet spread between humans, but he has warned that health authorities must be ready for the virus to mutate at any time.

"It looks preventable and controllable now, and there is no evidence showing human-to-human transmission," Li said.

However, China reported 10 more cases of the avian influenza in the space of just three days, bringing the total number to 122 including 24 deaths, according to reports.
Li urged health officials to lower the rate of fatalities from the disease and to continue releasing information to the public, it emerged.

He spoke with officials from Zhejiang province, which has had 45 cases, as well as those from Lushan county in Sichuan province, the epicenter of the 6.6-magnitude earthquake on April 20.

In a study published online in The Lancet medical journal, Chinese scientists confirmed the H7N9 virus is transmitted to humans from chickens, confirming that up to now no evidence of human-to-human transmission has been found.

Last week, the World Health Organization described the virus as "one of the most lethal", and said it is more easily transmitted than an earlier strain that has killed hundreds globally since 2003. http://www.express.co.uk/news/world/395665/Chinese-Premier-calls-for-high-alert-as-number-of-bird-flu-infections-continues-to-rise

..CHINA'S Premier Li Keqiang yesterday urged authorities to be vigilant against the new strain of bird flu that has killed 23 people, while saying that efforts to tackle the virus have so far been effective.

Speaking during a visit to the Chinese Center for Disease Control and Prevention in Beijing, the premier warned people to prepare for new developments amid fears H7N9 could mutate into a form easily transmissible between humans.

"Countermeasures have been effective so far, but the situation is still developing as new cases turn up," Li said.

"We cannot afford to take it easy or relax, as we are facing a new virus," he said. "We should be prepared for any possible development."

He said more effort was needed to diagnose and treat people as early as possible.

Li pledged to spare no efforts in treating infected patients and reducing casualties.

"We will make sure no patients have to deal with delayed treatment because of costs," he said.

Li said the government will continue to cooperate with international organizations, release information openly and increase public awareness...

H7N9 Zaozhuang Shandong Cluster Raises Concerns

Patients Zhang, male, 36 years old, who lives in Zaozhuang City, and engaged in the work of building materials wholesale. Fever, cough 6 days, hold breath after 1 day the Zaozhuang City hospital on the evening of April 21, admitted to hospital with severe pneumonia

In Shandong, experts confirmed an H7N9 bird flu case in the city of Zaozhuang on Sunday. The patient, a four-year-old boy surnamed Zhang, developed a fever on Saturday.

The above translation (in red) describes the first confirmed H7N9 case(36M) in Zaozhuang, Shandong Province, who developed symptoms on April 15 (see map).  The comments (in blue) describe the confirmation of H7N9 in his son (4M), who developed symptoms on April 27.  The 12 day gap in disease onset dates strongly signals human to human transmission.  Similar time gaps exist for family members in other H7N9 bird flu clusters, including the Minghan familial cluster where disease onset dates between son and father was 8 days (both cases died).

Although the disease onset date gaps provided compelling evidence of human yo human (H2H) transmission, the evidence for sustained H2H transmission is the identity in three genes found on A/Hangzhou/1/2013 and A/Jiangsu/01/2013, which included L226I

The dramatic spread of confirmed cases in recent days (see map) also supports widespread H

7N9 bird flu virus could mutate – expert

wASHINGTON: There is no evidence that the deadly H7N9 bird flu has yet spread between humans in China but health authorities must be ready for the virus to mutate at any time, a top United States virologist has warned.
Anthony Fauci, the head of the National Institute of Allergy and Infectious Diseases, said officials in China had studied more than 1,000 close contacts of confirmed cases and not found any evidence of human-to-human transmission.

“That is powerful evidence because if you had a thousand contacts with someone with the flu you would be pretty sure some of them would have been infected,” Fauci said in an interview with Agence France-Presse.

Nevertheless, Fauci cautioned that authorities needed to be ready for the possibility of the virus mutating and spreading between humans.

“It’s unpredictable as are all the influenza. One of the things we need to be concerned about is this might gain the capability of going human-to-human which up to this point has not happened and is somewhat encouraging news,” Fauci said.

“But we still need to be very prepared for the eventuality of that happening.”

Researchers are already developing a diagnostic test to identify H7N9, along with a vaccine, with clinical trials due in July or August.

“Work is under way on making a diagnostic test to be able to pick it up quickly,” Fauci said.

“We have already started on an early development of a vaccine as we did with H5N1 years ago . . . Hopefully, we will never have to use it.”

More than 110 people in mainland China have been confirmed to be infected with H7N9, with 23 deaths, since Beijing announced on March 31 that the virus had been found in humans.

Most of the cases have been located in eastern China, although Taiwan has reported one case. Another case has been found in southern China, while Chinese officials confirmed a further outbreak in the central province of Hunan.

Chinese authorities have identified poultry as the source of the virus and have confirmed that patients became sick from contact with infected live fowl.

A visiting team from the World Health Organization, which wrapped up a week-long visit to China on Wednesday, said there had been no human-to-human transmission but warned H7N9 was “one of the most lethal” influenza viruses ever seen.

Fauci praised Beijing for its handling of the current crisis, contrasting it to the response of the outbreak of severe acute respiratory syndrome in 2002-2003, when China stood accused of covering-up the scale of the crisis.

“It was not the case with SARS in 2003 but the transparency has been excellent,” Fauci said. “I am quite satisfied with the Chinese response.”

Fauci likened the current H7N9 strain of bird flu “in some respects” to the H5N1 bird flu strain of several years ago.

“The similarities are that it is fundamentally a chicken or bird flu that jumps from chicken to humans and is quite severe when it infects humans,” he said.

However, Fauci added: “The difference between H7N9 and H5N1, is that H5N1 kills chickens very rapidly so it is easy to identify where the infected flocks of chickens are. H7N9 doesn’t make the chicken sick, so it has been difficult to pinpoint where the infected chickens are.

There have been 566 confirmed cases of the H5N1 strain of bird flu, which killed 332 people in the world—a mortality rate of 58 percent, compared to 20 percent for the H7N9 bird flu strain. The H1N1 “swine flu” pandemic o 2009, which appeared in Mexico at the same time of year as the H7N9, eventually infected 60 million people throughout the world and killed more than 12,000.

The 1918 Spanish flu, which has been called one of the deadliest plagues in human history, had a mortality rate of only two percent. http://www.manilatimes.net/index.php/news/world/46370-7n9-bird-flu-virus-could-mutate-expert?

H7N9 L226I Spread Raises Pandemic Concerns

Recombinomics Commentary 11:30
April 28, 2013
The WHO Chinese National Influenza Center have released three sets of sequences (H7, N9, MP) from two cases in Shanghai (A/Shanghai/3/2013 and A/Shanghai/4/2013), and one from Jiangsu (A/Jiangsu/01/2013).  They are to be commended for the rapid release of these important sequences. The two cases from Shanghai had collection dates in late February and early March, suggesting they are from the cases from the Fifth Peoples Hospital in the Minhang District (see map).  Both had H7 Q226L, which is common and bird flu H7N9 sequences from avian and human sources.

However, the Jiangsu sequence, A/Jiangsu/01/2013, has L226I, which was also present in A/Hangzhou/1/2013, which was from a chef who worked in Jiangsu, where he was likely infected.  Moreover, all three sequences from Jiangsu/1 exactly matched the Hangzhou/1 sequence signaling clonal expansion.  The full Hangzhou/1 sequence also had PB2 E627K.

L226I is present in seasonal flu, and signals a human adaptation.  It has not been reported in any of the avian sequences, and its presence in identical human sequences raises serious pandemic concerns.

Poultry firm owners suffering from flu to notify hospital of profession

TAIPEI, Taiwan -- The Council of Agriculture (COA) yesterday demanded poultry business owners who are suffering from flu symptoms to seek medical treatment immediately and notify the hospital of their profession so that the relevant authorities can conduct H7N9 influenza inspections more easily.Huang Guo-ching (黃國青), the deputy director of the COA, said that the council will also double the number of random inspections on migratory birds and fowl from 12,000 to 24,000 annually.

“H7N9 does not produce any symptoms in fowl, so the COA is taking more precautions to prevent a potential influenza outbreak,” Huang said.
“The COA would also like to ask poultry business owners to go to a doctor immediately when they start to show flu symptoms, instead of waiting until the condition worsens,” the deputy director added.
“It is important to let medical personnel know that the patient is in the poultry business so that they can run a test to verify if the patient is suffering from H7N9,” Huang said.
The COA can also track the source and make sure if the location in which the patient keeps poultry or fowl contains the H7N9 influenza virus, according to Huang.
Poultry-related businesses, Huang added, include people who keep, transport, cook, or sell fowl and poultry...
