China to test entire country for deadly new flu

The World Health Organization says China is broadening its search for human cases of the new H7N9 flu.
A spokesperson for the Geneva-based global health agency says China is starting to test for the new strain across the country.
To date cases have been found only in four provinces in eastern China, near Shanghai.
But Gregory Hartl says Chinese authorities are starting to test for the virus through the country’s surveillance network for influenza-like illnesses.
That means people sick with flu-like illnesses who seek care at sites in the network across China could be tested for H7N9.
SEVEN DEATHS SO FAR
To date China have confirmed 24 infections, with seven deaths.
It’s not clear whether the clustering of cases in the four provinces means that those are the only places where infections are occurring, or whether broadening testing will turn up cases in more far-flung parts of the vast country.
ANIMALS TESTED
Hartl said the work to find the animal reservoirs of the virus is also continuing. China’s Agriculture Ministry and the UN Food and Agriculture Organization are collaborating, he said, and have collected more than 11,000 samples from various animal types to look for where the virus is hiding in nature.
So far H7N9 has been found in pigeons, quails and chickens.
Finding the virus in animals has been challenging because it currently doesn’t sicken or kill poultry.
H7 flu viruses, like the better known H5 viruses, can come in two forms. They can be highly pathogenic, killing poultry, or can come in a low pathogenicity form. The new virus is the latter, meaning it can spread in a virtually invisible manner among poultry flocks.
However, low path H7 and H5 viruses will typically evolve to become high path viruses, if they circulate among poultry for long enough. (The designations of low path or high path relate to how the viruses act in poultry, and not how deadly — or not — these viruses are for people who become infected with them.)
HUMAN INFECTIONS CONTINUE
Hartl says human infections continue to accumulate at a rate of two or three a day, but the WHO believes the cases are still sporadic, with “almost no evidence of human-to-human transmission of any type.”
He explained he used the word “almost” because among the more than 600 contacts of cases that China is investigating, a few have shown signs of illness and need to be followed up.
HUMAN TRANSMISSION
At this point there may have been rare occasions of limited human-to-human spread, of the type that is occasionally seen with H5N1 bird flu, he acknowledged. Small clusters of H5N1 cases have occurred, but only among people who have had close contact with someone ill with the virus.
With H7N9, “there is absolutely no evidence of any sustained human-to-human transmission,” Hartl says.

But chairman of the Influenza Specialist Group, Alan Hampson, says the strain has traits which will make it transferable between humans.
"It does appear to have some of the adaptive changes that might make it a humanised virus," he said.
"Another concern is that it's a virus which, unlike a lot of other viruses and the H5 virus, doesn't appear to be making people sick.

7 APRIL 2013 - As of 7 April 2013 (16:30 CET), the Chinese health authorities notified WHO of an additional three laboratory-confirmed cases of human infection with influenza A(H7N9) virus.
The first patient is a 59-year-old man resident of Shanghai, who became ill on 25 March 2013, and is now in critical condition. The second patient is a 55-year-old man from Anhui who became ill on 28 March 2013, and is now in stable condition. The third patient is a 67-year-old man from Shanghai who became ill on 29 March 2013, and is considered a mild case.
To date, a total of 21 cases have been laboratory confirmed with influenza A(H7N9) virus in China, including six deaths, 12 severe cases and three mild cases.
More than 530 close contacts of the confirmed cases are being closely monitored. In Jiangsu, investigation is ongoing into a contact of an earlier confirmed case who developed symptoms of illness.
The Chinese government is actively investigating this event and has heightened disease surveillance. Retrospective testing of recently reported cases with severe respiratory infection may uncover additional cases that were previously unrecognized. An inter-government task force has been formally established, with the National Health and Family Planning Commission leading the coordination along with the Ministry of Agriculture and other key ministries. The animal health sector has intensified investigations into the possible sources and reservoirs of the virus.
WHO is in contact with national authorities and is following the event closely. The WHO-coordinated international response is also focusing on work with WHO Collaborating Centres for Reference and Research on Influenza and other partners to ensure that information is available and that materials are developed for diagnosis and treatment and vaccine development. No vaccine is currently available for this subtype of the influenza virus. SHANGHAI, April 7 (Xinhua) -- China reported three more H7N9 infections on Sunday, bringing total number of the confirmed cases to 21 as the country is gearing up to fight the disease that has left six dead.
The latest confirmed H7N9 case was reported in east China's Anhui Province, where a 55-year-old male working in the live poultry trade was diagnosed with the virus, local health authorities said on Sunday night.
The Chinese Center for Disease Control and Prevention (CCDCP) diagnosed the patient, surnamed Li, positive of the H7N9 avian influenza virus on Sunday.
Li began exhibiting flu symptoms on March 28 and was taken to a hospital in Bozhou City on April 1 after his condition worsened, according to a statement from the Anhui Provincial Health Department.
Li is currently in stable condition, said the statement, adding that 12 others who have had close contact with the patient have not shown any signs of infection so far.
Before Li's case of H7N9 was confirmed, the National Health and Family Planning Commission (NHFPC) reported that there were a total of 20 H7N9 cases across the country as of 5 p.m. Sunday, including two cases that were newly reported in Shanghai on Sunday.
One is a Shanghai resident and the other is from neighboring Anhui Province, according to Shanghai authorities.
The two male patients, 67 and 59 years old, respectively, developed flu symptoms in late March and were diagnosed with pneumonia over the last week, Shanghai's health and family planning commission said in a statement.
An initial investigation showed that six people who have had close contact with the two have not exhibited flu symptoms.
So far, Shanghai alone had 10 confirmed H7N9 cases, with four resulting in death. The other six patients have been quarantined and are undergoing treatment.
As of 5 p.m. Sunday, three cases, including two that ended in death, had been reported in east China's Zhejiang Province, six cases in Jiangsu Province, and one case in Anhui Province, according to the NHFPC.
Governments at provincial levels across the country have reinforced efforts in their fights with the disease, even if there are no such reports in some regions.
Authorities in Shanghai have placed 194 people who had close contact with the 10 infected people under medical observation. Two have exhibited symptoms, although bird flu has been ruled out as the cause of their illness.
To prevent the spread of the virus, Shanghai authorities have ordered the slaughter of 98,000 poultry and incinerated them, according to the municipal agriculture commission.
Shanghai Party chief Han Zheng vowed on Sunday to implement all preventative and controlling measures against the bird flu in an efficient and transparent way.
In Beijing, where there is no H7N9 case reported so far, the authorities have banned all live poultry trade outside the market and unlicensed butchering of poultry.
The government of south China's Guangdong Province has earmarked an initial 30 million yuan (4.76 million U.S. dollars) as fund for the prevention and control of the disease as well for aiding infected patients in poverty.
Testing reagents for the H7N9 avian influenza virus have been distributed to 409 flu monitoring sites across the country, according to a statement by the CCDCP.
The center has also provided major infectious disease hospitals and research agencies with testing materials and has provided instructions for conducting the tests, it said.
The center said it has participated in a series of teleconferences organized by WHO headquarters and has reported on the H7N9 infection situation in China in a timely manner.
The center is also maintaining communication with health authorities in the United States and other countries regarding technical cooperation.
On Saturday, China approved a new drug believed to be effective in treating the H7N9 avian flu virus.
