Thailand-Alert for H7N9 birdflu virus after deaths in China

The Public Health Department has set up nationwide surveillance for H7N9 bird-flu virus after three people in China were infected and two of them died.

The system includes a lab test for confirmation, Surveillance Rapid Response Teams (SRRTs) and a check at hospitals around the country for patients with symptoms similar to severe pneumonia and flu, Public Health Minister Pradit Sinthawanarong said Monday.

Suspect Shanghai H7N9 Familial Cluster

Recombinomics Commentary 03:00
April 1, 2013
The 87-year-old's two sons have gone to hospital with similar symptoms.
According to Shanghai No. 5 People's Hospital, the three members of the Li family were admitted between February 14 and 24 for symptoms including a high fever and coughing. All three were diagnosed as having pneumonia. The 69-year-old son recovered and was discharged but the 55-year-old died from severe pneumonia and respiratory failure in late February. The father died of multi-organ failure.

The above comments describe family members of the first confirmed case (87M, A/Shanghai/1/2012) who developed pneumonia.  The confirmed case, as well as one of his sons, died.  The cluster raises concerns that H7N9 is transmitting human to human (H2H) which is not being identified with current testing procedures.  It is likely that all three were H7N9 infected.  The sequence for A/Shanghai/1/2013 was more diverse than A/Shanghai/2/2013 and Anhui/1/2013, but all three had PB2 E627K as well as the 15 BP deletion in N9, signaling human adaptation.

Disease onset dates for the sons would be useful.  H7 has also been identified in horses, supporting mammalian transmission, and other human cases have been associated with symptomatic contacts.  Serological studies had also signaled human transmission, although prior clusters have been mild.  The only prior fatal H7 case (H7N7 in Netherlands in in 2003) had PB2 E627K, which was present in all three 2013 confirmed H7N9 cases, which were severe or fatal.

Chinese avian flu deaths in the South Korean government to increase inspection efforts

According to Yonhap April 1 news, South Korean Agriculture and Forestry Livestock Food Ministry on the 1st said, recently China has three people infected with H7N9 avian flu virus, two of whom died, on this South Korean government will strengthen inspection and quarantine work.
Reported that Prior H7N9 avian flu found in birds between human infection of H7N9 avian influenza is the first time. So far, the cause of human infection and lethal avian influenza H5N1 virus.
The South Korean government decided to strengthen the occurrence of cases of livestock diseases tourists inspection visit and tourists to visit these countries try to avoid visiting livestock farms, livestock farmers will have to do the prevention work and limit external persons or vehicles entering and leaving. Korea Agro Food Ministry official asked, if the cattle suspected to be infected with avian flu, should immediately reporting to the relevant livestock epidemic prevention institutions.

Questions in China on how H7N9 flu strain killed 2

Apr. 1, 2013 7:34 AM ET

(AP) — Health officials say they still don't understand how a lesser-known bird flu virus was able to kill two men and seriously sicken a woman in China, but that it's unlikely that it can spread easily among humans.
Two men in Shanghai became the first known human fatalities from the H7N9 bird flu virus after contracting it in February. A woman in the eastern city of Chuzhou remains in serious condition, China's National Health and Family Planning Commission said.
It was unclear how the three patients became infected, the health agency said. It sought to calm fears about the virus but provided few details about each case. Authorities have not described the patients' occupations or said whether they had come into contact with birds or other animals.
The health authority noted, however, that two sons of one of the Shanghai men also suffered from acute pneumonia, and the source of their infection is still unknown. The Chinese health agency said other people who were in close contact with the victims have not become sick, indicating that the virus is not easily transmitted between humans.
"We don't know yet the causes of illness in the two sons, but naturally, if three people in one family acquire severe pneumonia in a short period of time, it raises a lot of concern," the World Health Organization's China representative, Michael O'Leary, said at a briefing in Beijing late Monday.
Other strains of the H7 family of bird flu viruses have caused mostly mild human infections in the past, said University of Hong Kong microbiologist Malik Peiris, with cases reported in the Netherlands, Canada, the U.S. and Britain — mostly following outbreaks in poultry.
Experts say the deaths in China might indicate that the H7N9 strain has morphed to become more lethal to humans, although it's not possible to make any conclusions yet about its mortality rate because many mild cases may go undetected. A thorough tracing of the virus is critical.
"I would guess that given the severity of the human disease it is likely that these particular viruses have undergone the change to become highly pathogenic but obviously that remains to be ascertained," Peiris said. "The crucial question is the source of this virus, where is it."
Scientists have long feared that another bird flu strain, H5N1, might mutate to spread more easily. But while it has decimated poultry stocks mostly in Southeast Asia, it has only occasionally sickened people — mostly after close contact with infected poultry. It can be deadly when it does infect humans, though, killing about 60 percent of the time.
"At the moment, there has been not much evidence of human-to-human transmission (of H7N9) so to that extent it is similar to the H5N1 situation, but it is early days and so there's a lot more to be understood," Peiris said.
More than 16,000 pig carcasses were fished out of the river system that supplies some of Shanghai's water supply in March, apparently dumped by farmers after they were sickened, and some observers have wondered whether there might be a link between the sickened pigs and the bird flu deaths.
However, O'Leary said the H7N9 infections pointed to an avian flu, not a pig disease, so he said he thought the timing of the pig deaths was coincidental. But he said that epidemiologists were looking at any possible avenue of infection.
Scientists classify flus based on the proteins on the surface of the virus: There are 17 varieties of hemagluttinin, the H in a flu's name, and 10 varieties of neuraminidase, the N component. Any combination of those Hs and Ns could crop up and potentially mutate into a form that's spread easily from person to person, making it dangerous enough to produce a pandemic.
Health authorities are monitoring 88 people who came into contact with the H7N9 patients and have not found any additional infections so far, China's health agency said. Experts say that indicates that the chance of human-to-human transmission is low.
"It is very unlikely, because the virus has to break the species barrier and this is usually quite a difficult event. There has to be a lot of significant mutation," said David Hui, an infectious disease expert at the Chinese University of Hong Kong.
Many epidemiologists regard densely populated parts of China and Southeast Asia, where farmers often live in close quarters with pigs and poultry, as regions where conditions are ideal for nurturing infectious diseases that jump from animals to humans. An earlier deadly outbreak of SARS, or severe acute respiratory syndrome, was linked to wild animals that infected animals, which in turn infected people in that region.
One of the male victims of H7N9, who was 87, became ill on Feb. 19 and died on Feb 27. The other man, 27, became ill on Feb. 27 and died on March 4, the Chinese health commission said. A 35-year-old woman in the Anhui city of Chuzhou became ill on March 9.
Timothy O'Leary, the World Health Organization's regional spokesman in Manila, said the risk to public health appears low because there is no evidence of human-to-human transmission, but that WHO is closely monitoring the situation.
"Obviously we're very concerned about the evidence that humans have not only become infected with H7N9, but also have died as a result of it," O'Leary said. "So we're taking it seriously." http://hosted2.ap.org/APDEFAULT/bbd825583c8542898e6fa7d440b9febc/Article_2013-04-01-China-Bird%20Flu/id-415cf6848dfc40679168f4c88c41baf3
Shanghai Municipal Health Bureau to inform the media last night: the morning of March 30, the experts of the National Health and Family Planning Commission, Shanghai, Anhui, eight cases of unexplained pneumonia cases discussed diagnosed, Shanghai and Anhui Province found three cases of human infection with the H7N9 avian influenza. Medical observation, it is learned, the influenza virus subtype previously in Europe, the Americas and Asia, Mongolia, South Korea in birds, which is at home and abroad for the first time found that human infections.
Medical observation, so far, all close contacts of the cases were not found similar symptoms and morbidity. This year the city of influenza, pneumonia, the overall incidence of stable and no significant increase compared with the same period in the last three years the incidence signs. The city health department has to further strengthen the monitoring of respiratory infectious diseases prevention and control.
The Minhang and his son has the incidence of death
February 19, February 27, Lee, Wu, Minhang a father and son in the city among the two patients, respectively, due to the occurrence of symptoms such as fever, cough treatment in the Fifth People's Hospital of Shanghai. Among them, 87-year-old father and 27-year-old son was diagnosed with severe pneumonia, respectively, on March 4, after she died on March 10. Dead early onset of both influenza-like symptoms, a week after symptoms develop into pneumonia and was progressively increased, two patients had chronic underlying disease history. Lee before the onset of his two sons are also due to similar symptoms to the hospital for treatment, the eldest son was diagnosed with a lung infection, pneumonia, has been cured; youngest son was diagnosed with severe pneumonia after she died.
In the early morning of February 26, Minhang District Health Bureau then City Hospital reported a 3 District Disease Prevention and Control Center after the pneumonia admission "news, immediately organized to carry out epidemiological investigation, laboratory testing and expert consultation, to carry out the treatment job. Municipal Health Bureau immediately organized the Municipal Center for Disease Control and Minhang District Health Bureau and other units rushed to the City Hospital to conduct on-site investigations, rule out infectious atypical pneumonia, avian influenza H5N1 and novel coronavirus might.
Since mid-February to 5 March, City Hospital has public health to the City Center and the City of clinical disease prevention and control center for inspection seven cases of pneumonia cases (including the family of three cases) specimens. Have seven cases of pneumonia cases early onset of flu-like symptoms, including six cases of severe cases a week after the onset of symptoms develop pneumonia and was progressively increased, five cases of rapid development of acute respiratory distress syndrome (ARDS) and respiratory failure and death. deaths have chronic underlying disease history of alcoholism, smoking and other unhealthy habits, in some cases. The remaining 2 cases were discharged.
Hospital has public health to the City Center and the City of clinical disease prevention and control center for inspection Lee Wu from patients, the Municipal Public Health Clinical Center for Disease Prevention and Control Center in a timely manner to carry out the relevant laboratory screening, has ruled out infection seasonal flu, H1N1, people infected with the highly pathogenic avian influenza (H5N1) and SARS, novel coronavirus might further testing, found that patients may be infected can not be sub-type of influenza virus. According to City Public Health Center and the City CDC test results in censorship cases samples detected five cases of suspected influenza virus H7 gene fragment. After the China Disease Prevention and Control Center further testing to confirm the two deaths have been diagnosed as infected with influenza A H7N9 avian influenza virus, the remaining five cases diagnosed and troubleshoot the need for further support of the pathogen detection.
The city closely monitoring respiratory infectious diseases
It is understood that since the beginning of this year, the city of influenza, pneumonia incidence is generally stable, compared with the same period in the last three years, the incidence was no significant increase. The health sector has been further strengthened monitoring prevention and control of infectious diseases of the respiratory tract.

Hong Kong issues warning on new bird flu strain

HONG KONG: The Hong Kong government is stepping up inspection at border checkpoints, as news of at least two deaths from a new strain of avian flu in mainland China reverberates in the city.
Health Secretary Ko Wing Man on Monday issued warnings about H7N9, saying he is concerned that the virus had mutated and could be spread between humans. He also said that he is seeking more information from mainland health authorities about the strain which has been found in humans for the first time.
The Chinese government said over the weekend that two men, one aged 87 and the other 27, had died in Shanghai after being infected with H7N9. A 35-year-old woman in the eastern province of Anhui, near Shanghui, is in critical condition.

The Secretary for Health, Ko Wing-man, has said he is seeking more information from mainland health authorities about a deadly strain of avian flu that has been found in humans for the first time. This follows the deaths of at least two people in Shanghai after they contracted the H7N9 bird flu virus.
Dr Ko said he was concerned about whether the virus had mutated and could be spread between people.
The Centre for Health Protection said it was stepping up surveillance for suspected cases.
The former health chief, York Chow, warned it was very likely that the virus had mutated, as H7 itself is a low pathogenic form.
He said the government must not take the matter lightly because of the high fatality rate.

Shanghai infected with the H7N9 avian influenza family of the deceased had similar symptoms

87-year-old Lee and his two sons, in mid-February, has similar symptoms such as fever, cough, and was sent to Shanghai Fifth People's Hospital of Respiratory Medicine diagnosis and treatment.
Mr Lee, father and 55-year-old son was diagnosed with severe pneumonia, after she died, the 69-year-old son for a lung infection, has been cured. Father surnamed Li was diagnosed with the H7N9 avian influenza virus infection, and his two sons have not yet detected the H7N9 avian flu. For a family of three similar symptoms before whether contact with poultry, the hospital said hospital fever has set a special pre-screening at patients with symptoms associated with respiratory tract infection and fever, epidemiological history questionnaire will focus on and fill .
Was reported that another 27-year-old H7N9 avian flu patients pork vendors Nanjing involved in the consultation Anhui H7N9 avian flu patients, public health experts Lu Hongzhou this station, Anhui patients Mr. Han has contact with birds and pigs, there had been a history of exposure to poultry and pigs,

The world's first human-to-human transmission of H7N9 avian flu in Anhui dead a crisis now

The world's first human-to-human transmission of H7N9 avian flu in Anhui dead a crisis now

First identified human infection with the H7N9 avian flu cases in the world .National Health and Family Planning Commission informed yesterday (March 31), Shanghai City and Anhui Province, found three cases of human infection with the H7N9 avian flu cases. Two patients died, Anhui Ji patients in critical condition, Nanjing stay in the hospital for rescue. But informed sources, the TheShanghai total of four patients died, including a father and son from the same family. The experts pointed out that there is no targeted vaccine, no obvious indication that the virus with human-to-human transmission capacity.
Hong Kong Wen Wei Po reported, a notification from the National Health Committee, said Shanghai Municipal patients Lee, 87 years old, 19 February onset actively rescue died March 4. Patients Wu, Shanghai City, 27 years old, the incidence February 27, to actively rescue died on March 10. Patients Chuzhou City, Anhui Province, Mr. Han, female, 35 years old, March 9 incidence is currently in critical condition, Nanjing, Jiangsu active treatment. It is reported that the three cases the clinical manifestations are the early symptoms of respiratory tract infection such as fever, cough, and then developed severe pneumonia and breathing difficulties.
Informed sources, So far, Shanghai has a total of 4 patients with similar symptoms to death, four dead were in the period from February to March this year, admitted in Minhang District of Shanghai, Shanghai Fifth People's Hospital. The hospital internal sources, before and after the Lunar New Year, one surnamed Li patients to the hospital, since there are two named Li patients to see a doctor, the three patient relationship as father and son, respectively a doctor's time apart from each other two or three days. 3 among the oldest 87-year-old (communications consistent with the National Health Committee), the other two are 69 years old and 55 years old. Since then, the hospital has admitted another 27-year-old Wu, patients (in line with the National Health Committee communications), but not a kinship with the foregoing surnamed Li.
Allegedly, several patients initially are fever, cough, but her condition all the way to nosedive until the final due to severe pneumonia, respiratory failure and death. The basis of this one surnamed Li family of three all sick to death of this argument is different from the official statement yesterday.
During the several patients sick, the hospital and the Health Bureau, the CDC and other organizations the many consultation, but the test results are not pointing to a known infectious diseases. For insurance purposes, the hospital-level measures to deal with SARS "(SARS), and isolated these several patients in the hospital, the medical staff of all incoming and outgoing, are armed to prevent infection. That time, the hospital is up and down, the atmosphere was very tense.
The aforementioned hospital internal sources, these four patients are ordinaryShanghai locals, surnamed Li, a family of three, has retired. As for the patients with the death of a 27-year-old Wu, rumored career pork street vendors.
National Health Commission means belonging to the avian influenza virus , theH7N9 influenza virus infection from the virus biology, three cases of patients diagnosed with the specific source of the infection is not clear, but so far, the epidemic was not found between the three cases Diseases associated. In addition, 3 cases of 88 close contacts were not unusual. Currently, the Group has been rushed to a local guide emergency response work, Shanghai , Anhui , Jiangsu provincial and municipal health departments to track patients and close contacts and medical observation.
The experts pointed out that the H7N9 subtype of avian influenza virus previously only found in poultry between never found human cases, the people infected with the H7N9 avian influenza virus, is the world's first discovery of a new subtype of influenza virus. No targeted vaccine.
It is reported that the situation of the above cases timely notification to the World Health Organization, Hong Kong, Macao and Taiwan regions and countries concerned.

Notification of three human cases of H7N9 in Shanghai and Anhui

The Centre for Health Protection (CHP) of the Department of Health received notification from the National Health and Family Planning Commission (the Commission) today (March 31) concerning three confirmed human cases of influenza A (H7N9).

A CHP spokesman said the two cases in Shanghai were two men aged 87 and 27, who passed away on March 4 and 10 respectively. As regards the case in Anhui, the 35-year-old female patient is now in critical condition.

Laboratory tests on the three patients' specimens by the Mainland health authorities yielded a positive result for H7N9.

According to the Commission, the 27-year-old man was a butcher while the 35-year-old woman had exposure history to poultry before the onset of symptoms. No epidemiological link between the three cases was identified. So far, no abnormality was detected among the 88 close contacts of three cases.

The spokesman said that influenza A (H7) is a statutorily notifiable infectious disease in Hong Kong. The CHP is maintaining close liaison with the Mainland health authorities to obtain more information on the cases.

"We will heighten our vigilance and continue to maintain stringent port health measures in connection with this development," the spokesman said.

The spokesman reminded members of the public to remain vigilant against avian influenza infection and to observe the following measures:

* Avoid direct contact with poultry and birds or their droppings; if contacts have been made, they should wash hands thoroughly with soap and water;
* Poultry and eggs should be thoroughly cooked before eating;
* Wash hands frequently;
* Cover nose and mouth while sneezing or coughing, hold the spit with tissue and put it into covered dustbins;
* Avoid crowded places and contact with sick people with fever;
* Wear a mask when you have respiratory symptoms or need to take care of patients with fever; and
* When you have fever and influenza-like illnesses during a trip or when coming back to Hong Kong, you should consult doctors promptly and reveal your travel history.

For further information on avian influenza, please visit the CHP's website (www.chp.gov.hk).

New bird flu strain kills 2 patients in Shanghai

TWO men suffering from a lesser-known type of bird flu have died in Shanghai and a woman in eastern Anhui Province is in a critical condition, health authorities said yesterday.

The three cases of H7N9 avian influenza infection are the first time the virus has been detected in humans, the National Health and Family Planning Commission said yesterday.

So far there is no vaccination for the virus, it said.

The men were an 87-year-old surnamed Li who took ill on February 19 and died on March 4, and a 27-year-old surnamed Wu who became ill on February 27 and died on March 10.

A 35-year-old woman surnamed Han from Chuzhou City in Anhui became ill on March 9 and is being treated in Nanjing, capital of Jiangsu Province.

All three showed initial symptoms of fever and coughing which developed into severe pneumonia with difficulty in breathing in later stages, according to the commission.

There was no sign that any of the three had contracted the disease from each other, and no sign of infection in the 88 people who had closest contact with them.

H7N9 bird flu is considered a low pathogenic strain that cannot easily be contracted by humans. The overwhelming majority of human deaths from bird flu have been caused by the more virulent H5N1, which decimated poultry stocks across Asia in 2003.

The World Health Organization is "closely monitoring the situation" in China, regional agency spokesman Timothy O'Leary told the Associated Press.

"There is apparently no evidence of human-to-human transmission, and transmission of the virus appears to be inefficient, therefore the risk to public health would appear to be low," O'Leary said.

The three cases were confirmed to be human infection with H7N9 avian influenza by a team of experts called in by the health commission.

On Friday, the Chinese Center for Disease Control and Prevention separated the virus from samples from the patients.

The commission said it was still unclear how the three got infected.

"So far, it is still an animal virus not a human virus," said Jiang Qingwu, dean of Public Health School of Fudan University.

Since only three cases of human infection of H7N9 have been found, relatively little research has been done on it. The expert team is working to study the toxicity and human-infection capacity of the virus, the commission said.

Yesterday, Shanghai Health Bureau said cases of flu and pneumonia in the city were normal for this time of the year.

Neither son had the H7N9 virus, the bureau said.
It has ordered local hospitals to step up monitoring and supervision on cases involving flu, pneumonia and other respiratory diseases.

Experts say there is no evidence to indicate the virus is spreading but patients with symptoms such as fever, coughing and breathing difficulties are being urged to visit their doctors, ensure good hygiene, such as by washing their hands, and avoid contact with diseased poultry and livestock.

all close contacts of patients implemented under medical observation

...
According to the relevant provisions of unexplained pneumonia Management, Shanghai, Anhui and Jiangsu, the health sector (1 patient in Anhui where Nanjing) all close contacts of patients implemented under medical observation. The Shanghai patients diagnosed patients prior to the onset, the two family members with severe pneumonia, the cause is still under investigation and detection. 3 patients the incidence of the family association, pending further investigation. In addition, Shanghai, Anhui diagnosed patients of all close contacts were not found in similar cases.
Second, the National Health and Family Planning Commission, as well as local health departments to take measures? Which investigations were carried out?
