Illness cluster linked to Disney tour baffles officials


Orange County health officials are investigating a cluster of illnesses linked to Walt Disney World's "Wild Africa Trek" experience, a boutique tour in which small groups of people get up-close access to some of the wildlife in Disney's Animal Kingdom and a catered meal served on the theme park's manmade savannah.

Investigators have documented "several dozens of cases" of illnesses among guests who took the Disney tour in June and July, said Dain Weister, a spokesman for the Orange County Health Department.
The source of the illness remains a mystery.
"It appears to be some kind of stomach bug," Weister said. "It could be food-borne, it could be water-borne, it could be something that's passed on person-to-person, it could be something that's picked up by surface."
Disney has taken several precautionary steps, including "deep cleaning" various surfaces that guests touch, distributing more hand sanitizers, and re-emphasizing hand-washing policies to guests and employees.
"We are working closely with the Orange County Health Department to review the situation," Disney spokeswoman Andrea Finger said...

The county Health Department was alerted to the outbreak on June 11. Weister said the agency immediately inspected the kitchen at Disney World where employees prepare the food served on the trek, but they found no problems.

....Investigators also began contacting guests who took the tour, beginning with those who did so during the first two weeks of June and the first week of July. A majority of the cases they found occurred during the first week in June, though a handful were also found in July, so investigators are now surveying people who took the tour in the latter half of June...

..One of those who posted said he or she was "extremely sick" by midnight the day after taking the tour, with vomiting that lasted for about five hours and pain, cramping and diarrhea that endured for more than a week. Another said his wife had to take him to the emergency room because of complications....

Thailand-BMA closely monitoring hand, foot and mouth disease #EV71

BANGKOK, July 25 -- Bangkok Governor Sukhumbhand Paribatra said the Bangkok Metropolitan Administration (BMA) has closely monitored the situation on hand, foot and mouth disease as there were 13,000 patients nationwide and some 3,000 in Bangkok were infected with the virus.

The governor on Tuesday inspected Wat Ratchanadda School under BMA supervision to ensure the measures to prevent the spread of hand, foot and mouth disease are being strictly observed.
The BMA has instructed the schools to screen students everyday, to clean teaching aid equipment and toys. Schools were advised to educate students on how to improve personal hygiene and to wash their hands properly.
Mr Sukhumbhand said he was satisfied with the measures implemented in the city’s schools to prevent the spread of the disease and stressed that the public should not panic.
He said the BMA has closely monitored the situation during January-July and found that there were some 13,000 patients nationwide while in Bangkok alone, there were 3,000 patients.
The Department of Disease Control on Tuesday announced that a lab test confirmed a two-year-old girl died of a severe strain of hand, foot and mouth disease.
The toddler is the first HFMD death this year in Thailand. Recently, the international academic journals, Journal of Virological Methods, published online Chinese Academy of Sciences, Institut Pasteur of Shanghai Huang research group successfully prepared for the latest research results of the intestinal virus EV71 neutralizing monoclonal antibody.
The intestinal virus EV71 cause hand, foot and mouth disease pathogens. HFMD often occurs in children under the age of five infectious disease, some patients with severe neurological symptoms of encephalitis, pulmonary edema and paralysis or even death. In recent years, hand, foot and mouth disease in the Asia-Pacific region, widespread and poses a serious threat to the healthy development of children. In China, only the 2010 and 2011 the Ministry of Health statistics, on a total of 3,394,375 cases of infection, of which 1414 cases of death. Therefore, these monoclonal antibodies can be used to further the development of diagnostic kits for EV71. More importantly, in vitro experiments, the monoclonal antibody in very low concentrations can effectively prevent the virus from invading cells, suggesting that they have developed into a humanized monoclonal antibody drug for the treatment of the potential.

The study of early diagnosis of EV71 infection and monoclonal antibody drug development has laid a good foundation. The work was supported by the Chinese Academy of Sciences "Hundred Talents Program" funded.

CAMBODIA - Alert for an epidemic of foot and mouth disease that primarily affects malnourished children



Phnom Penh (Agenzia Fides) - According to some experts, the high rate of malnutrition in Cambodia has led to the rapid spread of an outbreak of foot and mouth disease that, since last April, has claimed the lives of 54 poor children . The "hidden hunger", lack of vitamins or minerals, is another serious problem in the country where half of all children under 5 years, the most vulnerable to HFMD, are anemic, mainly due to iron deficiency . Nearly 9 in 10 are already anemic year-old.



According to WHO, this year, there have been in China more than 1.27 million cases of mild and severe foot and mouth disease, with 356 deaths, compared with just over 711,300 cases last year. It also appears that the number of cases in Singapore has grown from 871 to over 26,000 in the first six months of this year. The WHO in Cambodia should be closed” is the emphatic sign-off to a lengthy and vivid analysis by Kantha Bopha Children’s Hospitals of the recent “mystery disease” scare.

While the World Health Organization and Ministry of Health have declared that a series of recent child deaths are linked to EV-71, causing severe hand, foot and mouth disease, Kantha Bopha founder Dr Beat Richner has slammed the conclusion as a “catastrophic declaration”.

Richner said 72 cases of the unexplained illness have appeared in his hospitals, and of those, 68 have died.

The WHO yesterday said the totals from the Ministry of Health stood at 61 cases and 56 deaths, including one new death over the weekend that fit the case definition of severe HFMD.

The “case definition” is the critical impasse between the MoH and WHO, and Kantha Bopha, which treat 85 per cent of all Cambodian children.

Kantha Bopha’s “case definition” is encephalitis and complete lung destruction leading to a rapid death.

Not one of these cases showed lesions associated with HFMD, Richner’s statement says.


However, WHO communications officer Sonny Inbaraj Krishnan told the Post yesterday that of the cases recorded by the UN agency and the MoH, rashes and blisters were present.

“Sometimes, with EV-71, either blister or rash appears and it could be one or the other,” he said. “The case definition [for WHO and MoH] is that the child has fever with neurological systems such as vomiting and prolonged stretches of sleepiness or convulsions and respiratory symptoms such as difficulty breathing.”

This conclusion is wrong and is still confusing the media and public, Richner rebuts in his statement.

The question that has still not been answered, Richner said, is “what is destroying the lungs in these 72 cases of encephalitis in the last 6 hours of their life, all treated in private clinics?”

Richner believes that the administration of incorrect drugs to these 72 children could be a factor in the astonishing destruction of the lungs present in the cases that have been recorded by his hospitals.

The most “catastrophic” press release by the MoH and WHO, according to the Swiss doctor, was the most recent, in which the joint investigation conclusion is that a mix of pathogens and a root affliction with EV-71 has been aggravated and worsened by the use of steroids.

The Post previously reported Richner decrying WHO’s criticism of the use of steroids as “absolute nonsense”.

The doctor emphasised that all patients arriving with the unknown illness at Kantha Bopha hospitals showed signs of encephalitis, which requires steroids to stem fatal swelling of the brain.

A high number of platelets during the final moments before death could be a sign that underlying tuberculosis is complicating treatment, Richner said in the statement.


Kantha Bopha Children’s Hospitals operate in parallel to the public health system in Cambodia and provide outpatient and emergency care free of charge at their five hospitals in Phnom Penh and Siem Reap.

However, a bitter relationship between Kantha Bopha and the WHO appears to be complicating investigations into and solutions to the child deaths.

MoH Secretary of State Heng Tay Kry said only the minister or WHO could respond about Richner’s allegations.


Minister Mam Bunheng, Communicable Disease Control Department director Sok Touch and his deputy, Ly Sovann, could not be contacted yesterday.

WHO country representative Pieter Van Maaren, told the Post he considered Richner’s statement “irrelevant”.

The Public Health Ministry has warned people against taking steroid-based medicines for suspected hand, foot and mouth disease (HFMD) without consulting doctors, saying the drugs could accelerate progression of the disease.

Certain drugs, especially those containing steroids, often make the patient initially feel better but eventually could result in the suppression of the immune system, Medical Services Department director-general Wilawan Juengprasert said Wednesday.

Steroid-based drugs suppress the immune system, which causes the disease progression to advance and could possibly lead to death, Dr Wilawan said.

She was speaking at a meeting of more than 500 paediatricians from both state and private-run hospitals.

The conference was organised by the ministry and aimed at updating medical specialists with the latest news of HFMD in the country.

Parents whose children are suspected of contracting HFMD are strongly advised to inform doctors, especially if their children have chronic diseases...

...As of Sunday, the accumulated number of patients diagnosed with HFMD in the country rose to 16,860, including a two-year-old girl whose death was confirmed on Tuesday to have been caused by the enterovirus 71 strain of the disease. The girl died at a hospital in Bangkok last week.

More confirmed HFMD infections were reported Wednesday, including four cases who were students of Thaworn Witthaya Kanchanaburi School in Muang district of Kanchanaburi.

Another case was a Mathayom 2 student of Suankularb Wittayalai School in Phra Nakhon district of Bangkok.

In Nakhon Ratchasima's Muang district, director of Wat Sa Kaeo School Suwat Boonyanuson dismissed rumours that a Prathom 1 student, who died on Tuesday after falling ill on July 16, had contracted HFMD.

Mexico to vaccinate 10 million birds in flu outbreak

MEXICO CITY — Mexico will start vaccinating some 10 million poultry Thursday against the highly contagious bird flu strain that has already led to the deaths of five million birds which either fell ill or were slaughtered.
"Starting tomorrow, we are going to vaccinate hens and chicks across the country to put an end to this bird flu epidemic," Mexican President Felipe Calderon said.
The president indicated that the ultimate goal is to have a batch of 80 million vaccinations, so that "in the coming weeks, (Mexico can) end this economic impact on poultry producers."

World's first dengue vaccine beats three virus strains

The world's first vaccine against dengue fever has protected against three of the virus's four strains in a keenly awaited clinical trial in Thailand.
French drugmaker Sanofi says the proof of efficacy is a key milestone in the 70-year quest to develop a viable dengue shot.
The company says the results also confirm the safety profile of its vaccine candidate.
The mosquito-borne disease is a threat to nearly 3 billion people and is caused by four different types of virus.
Sanofi's vaccine generated an antibody response for all four dengue virus types, but evidence of protection was only demonstrated against three of the four strains circulating in Thailand.

Swine Flu at an Indiana County Fair #H3N2v


The Indiana State Department of Health is investigating the potential outbreak of variant influenza A virus, or swine flu.
Shortly after the LaPorte County Fair, held from July 8 to July 14, four individuals reported symptoms of cough, fever and sore throat. Test results from the patients were positive for variant influenza A. To determine the source of the outbreak, swine from the fair are also being tested.
The State Department of Health is working with the Indiana Board of Animal Health and the LaPorte Country Health Department to collect information and determine the spread of the outbreak.
According to the Department of Health, each of the individuals had direct contact with swine, and each has since fully recovered.
Variant influenza viruses are called so because they do not occur seasonally, nor do they normally affect humans. Influenza A viruses regularly cause outbreaks in pigs. Symptoms of influenza A in infected pigs include, sneezing, respiratory distress, fever, depression or lack of appetite. When infected pigs and humans are in close proximity or direct contact, influenza can be transmitted. The symptoms of human infection with a variant influenza A virus are similar to seasonal flu symptoms (fever, coughing, nausea, lack of appetite, etc.). In rare and limited cases, variant influenza has spread from person-to-person.
Swine flu, or variant influenza A virus, infections are concerning because pigs are susceptible to avian, human and swine influenza viruses. If a pig is infected with influenza viruses from different species at the same time, the creation of a new virus is possible. This process, in which genes from different viruses mix to create a new virus, is called antigenic shift. The 2009 H1N1 pandemic was the result of human infection with a genetically different influenza virus, against which humans had no protection. Now, flu vaccines include protection against the H1N1 virus.
The Indiana State Department of Health recommends frequent washing of hands when working with animals. Additionally, the department cautions against eating around animals, particularly if you have a weakened immune system. La PORTE — Sick humans and pigs at the La Porte County Fair had one thing in common — the H3N2 virus.

Yesterday the Indiana Board of Animal Health and the Indiana State Department of Health determined that the same virus was responsible for both animal and human sickness at the annual event, which drew more than 20,000 people to the La Porte fairgrounds earlier this month.

According to a health department press release, four people associated with the fair were diagnosed with the illness after having direct contact with the animals. They have all since recovered.

Janelle Thompson, public information officer for the Indiana Board of Animal Health, said samples were collected from sick people and animals on July 16 after several respiratory illness complaints in both pigs and the children were reported.


Thompson classified the type A influenza as a zoonotic disease, meaning it can be transmitted from animals to humans or vice versa.

The health department release said human infections are more likely to occur when people are in close proximity to live infected swine, such as in barns and livestock exhibits at fairs. Influenza viruses are not transmitted by eating pork and pork products.

Thompson agreed.

"All the people who were diagnosed in this situation had direct contact with the swine," she said. "They were all in the barn working with the animals. At this time we have not heard of any fairgoers becoming ill. So pork is still completely safe to eat."

One parent associated with the swine program (who asked to go unnamed for fear of retaliation) said he had two kids sent to the emergency room, one with a temperature of 102 degrees and the other with a temperature of 104 degrees.

He explained that participants started noticing the problem on July 12.

He said he knew of 41 sick pigs. He owned one of them.

According to the health department, the reported symptoms included coughing, fevers and sore throats. It's continuing to collect information on the extent of the illnesses.

The department release said 17 human cases of this virus have been identified nationwide. Six of these cases have been identified in Indiana.

It recommended individuals to wash hands with soap and water before and after petting or touching any animal, and to never eat, drink, or put anything in their mouths in animal areas. And older adults, pregnant women, young children and people with weakened immune systems should be extra careful around animals.

INDIANAPOLIS—The Indiana State Department of Health, the Indiana Board of Animal Health, and the LaPorte County Health Department are actively investigating an outbreak of four human illnesses associated with the LaPorte County Fair, held July 8-14. All four individuals had direct contact with swine and all four cases are now recovered.


The State Health Department continues to collect information to determine the extent of the illnesses. Symptoms reported include cough, fever and sore throat. Confirmed test results on patient specimens indicate the cause as a variant influenza A virus. Swine specimens also tested positive for this virus.

Influenza viruses can be directly transmitted from swine to people and from people to swine. Human infections are most likely to occur when people are in close proximity to live infected swine, such as in barns and livestock exhibits at fairs. Influenza viruses are not transmitted by eating pork and pork products.

Since 2011, 17 human cases of variant influenza A virus have been identified nationwide. Six of these cases have been identified in Indiana.

Individuals should always wash hands with soap and water before and after petting or touching any animal. Never eat, drink, or put anything in your mouth in animal areas. Older adults, pregnant women, young children, and people with weakened immune systems should be extra careful around animals.

Hoosiers who have direct, routine contact with swine, such as working in swine barns or showing swine at fairs, and have experienced cough or influenza-like illness should contact their health care provider or local health department. Symptoms include cough, sore throat, fever, body aches, and possibly other symptoms, such as nausea, vomiting, or diarrhea.



While influenza is not an uncommon diagnosis in pigs, the State Board of Animal Health encourages swine owners to contact a veterinarian if their animals show signs consistent with flu, including coughing, respiratory illness, off-feed and fever. Most county fairs have a private veterinary practitioner on call for on-site assistance.

The Thai News Agency reports that as many as 14,000 people have been infected with Hand, Foot and Mouth Disease (HNFD) in an outbreak that is spreading across the region and the world. Reports say that the number of reported cases has fallen in the past week but is still worrying enough for the Prime Minister, Yingluck Shinawatra Yingluck, to have issued a statement saying that disease control measures are to be stepped up and that there must be increased vigilance.

In Cambodia, EV-71, a particularly virulent form of the disease, has killed at least 54 people but medical sources say that the number may have been increased by the innaproriate use of steroids in an attempt to cure what was, at first, an unidentified illness amongst children in a Phnom Penh hospital. Other non-fatal cases have now been identified. Most of the deaths were of infants of less than three years of age. To combat the spread of the disease, Cambodia closed schools in the affected area.
In the USA, doctors have reported a significant increase in the number of cases in Hartford, Connecticut. One hospital has reported seeing as many as nine patients in one day, most infants or young children.
The spread of the disease in Argentina has led to Japan introducing restrictions on imports from that country.
An outbreak in Sabah, in Malaysian Borneo, led to a one-week closure of schools and daycare centres in May this year and the outbreak appears to have been contained. In Sarawak, also in Malaysian Borneo, there were 806 cases in the first seven weeks of this year - almost four times the number in the same period in 2011. But the Malaysian Health Ministry said that the number did not qualify to be classified as an "outbreak." BANGKOK: Thailand has confirmed its first fatality this year from a virus that has killed hundreds of children across Asia.
The 2-year-old girl who died at a Bangkok hospital last week had a form of hand, foot and mouth disease known as enterovirus 71 strain, or EV-71. The disease is common among children ages 2 to 5 and is usually mild. But the severe form of the illness caused more than 400 deaths in the first half of this year in China, Vietnam and Cambodia.
Disease Control Department senior adviser Doctor Prasert Thongcharoen said Tuesday that laboratory tests showed the Thai girl was more vulnerable because she also had asthma.
The Public Health Ministry said about 16,000 children in Thailand contracted hand, foot and mouth disease this year.

Thailand- #EV71 case 'no cause for panic'

Department of Disease Control chief Dr Pornthep Siriwanarangsan yesterday advised the public not to panic over the discovery that a toddler had died at a Bangkok hospital from an Enterovirus 71 (EV71) infection.


Pornthep said the Nopparat Hospital had sent the virus found in the 2-year-old for analysis at the Department of Medical Sciences.

Pornthep advised parents to take early hospital treatment if they suspected their child was infected, otherwise it could be too late to fight it.

Department spokesman, Dr Wattana Uwanit, said officials are testing if the virus found in the body of the toddler had undergone a significant mutation that might cause it to become more viral...
